Please fill out the attached permit rescission form for NCG050007. Disregard any invoice you receive for this
permit. Budget usually sends out a past due after 30 days, please ignore. Mail the original signed form to the address
listed on the form. Also, you can scan a copy of the rescission form to me and I'll forward to the region to start the
process.
Also, the payment for NCG050430 was applied back in February.
Please let me know if you have any other questions.

There is only one site (Essentra Packaging 10500 Industrial Dr.). The other address is our PO box with our old name. So I
odn't think a transfer was intended.

Thank you for reaching out about this. I have forwarded the information to Laura Alexander so she can help us figure
this one out. It sounds like one of these needs to be rescinded, or perhaps it was supposed to be transferred to another
entity. Is that site still operating, or do you know it's no longer in need of a permit?

Per our conversation, I received 2 invoices for admin costs for a storm water permit.
The first one (permit# NCG050430) has our correct company name and address and was paid.
The second one (permit#NCG050007) has a previous company name (Keller Crescent) and a PO box that we use. I
believe this permit is not in use. Our accounting department may have paid it last year inadvertently.
Attached are the scans of invoices.
Please let me know what I need to do to make the Keller Crescent permit and invoice obsolete.
