April 4,2018 Mr.Darrell Gaines Cornerstone Conference Ministry and Retreat Center RECEIVE®/DENR/DWR P.O.Box 150 Browns Summit,NC 27214-0150 APR 11 2018 SUBJECT: Compliance Evaluation Inspection Water Resources Cornerstone Conference Ministry and Retreat Center WWTP Permitting Section NPDES Permit No.NC0046809 Guilford County Dear Mr. Gaines: On March 27, 2018, Jenny Graznak of this office met with Mark Nault (ORC) and Kris Pawlak (Lab Manager) of Meritech, Inc. to perform a Compliance Evaluation Inspection on the wastewater treatment system serving the Cornerstone Conference Ministry and Retreat Center. The inspection findings are detailed below and an inspection checklist is attached. 1.Permit The permit became effective July 1,2016 and shall expire April 30,2021.A copy of the current permit was available for review during the inspection. 2. Self-Monitoring Program The Discharge Monitoring Reports (DMRs) were reviewed for the period January 2017 through January 2018. There were three violations of permit limits during that time frame: one Fecal Coliform violation in March 2017 and two Aquatic toxicity violations in February and March 2017.All violations resulted in the issuance of Notices of Violation and were addressed in separate correspondence from this office.Regarding monitoring requirements, it was noted that sampling was performed per the frequencies specified in the permit. 3.Records/Reports A comparison of the March 2017 DMR with the corresponding laboratory records,data sheets,and operator records show that values were transcribed correctly onto the monitoring report. Records appear to be kept and maintained as required by the permit except for the lack of sludge removal records. Mr.Nault stated that Meritech does have those records, but they were not available during the inspection. Please make certain to have those records available at the next inspection. 4.Flow Measurement The permit requires weekly instantaneous flow reporting of the influent or effluent. The facility currently does not have a device to measure or estimate flow.Therefore,the operator performs a weekly bucket test. If the Retreat Center had a meter attached to the incoming water from the well, it could be used to estimate flow to the wastewater treatment system. However, it was determined during the inspection that no such meter exists on the premises. `-"T�dotlairra Compares State of North Carolina I Environmental Quality 450 W Hanes Mill Road,Suite 300,Winston-Salem,North Carolina 27105 Phone 336-776-98001 FAX.336-776-9797 5.Facility Site Review The existing 0.02 MGD treatment system consists of the following: splitter box(not used), dual in-ground 0.01 MGD package plants(only one unit is used due to low influent flow)with bar screens,aeration basins, clarifiers, and non-aerated sludge digester, tablet chlorinator with contact chamber, and a tablet dechlorination unit. A tertiary filter unit exists but is currently not used. 6; Operations and Maintenance The plant shows signs of age,wear and tear,and lack of structural repairs over the years.The wall between the aeration basin and digester does not appear to be secure.If there is any leaking between the units,solids cannot be concentrated in the digester nor liquid decanted. The wall should be checked to make sure it is secure and will allow for proper operation of the plant. 7. Sludge Handling and Disposal Very little solids are removed from this plant due to the low influent flow. Jenkins Waste Management removes solids when needed,but there were no records of this service available during the inspection. 8.Effluent/Receiving Waters The system discharges into an unnamed tributary to Benaja Creek,classified as WS-V NSW waters, in the Cape Fear River Basin. The outfall was buried in the stream bank and could not be located. The effluent after treatment appeared clear with no visible solids.
