Your cooperation during the site visit was much appreciated. Please advise the staff involved with this NPDES Permit by forwarding a copy of the enclosed report. The following areas of concern were observed during the inspection: 1) Facility has failed to conduct 24-hour flow based composite sampling as required by the permit. 2) Chain -of -Custody (COC) forms do not include the name of individual relinquishing the sample. Please be advised that this letter is a Notice of Violation. You are requested to respond to this Notice in writing within fifteen (15) days of receipt indicating your plan of action to ensure future compliance.
