Send all suspicious email as an attachment to ml_,Ixternal Andrew, These revisions began in December of 2015 when a large storm event dropped 12-18 inches of rain in a couple days in the Pacolet river watershed. The active stream restoration site did not have established vegetation and the storm resulted in large damages to the stream banks (see the attached pdf. The first photo was a few days before the storm and the second is the same spot a few days after). When we became aware of the damages and the need for changes to the design we corresponded with Mr. Zan Price by email and phone. The LPST and barbs were engineered to protect the very vulnerable banks in the damaged areas. This work was completed and stabilized rapidly to prevent further damage to the banks. As we entered the next phase of work we once again reached out to Mr. Zan Price in November of 2016 to notify him that changes would need to be made to the design due to changes in the site conditions. No revised design drawings were submitted for these changes. The barbs and LPST that were installed in previous phases of work have been functioning wonderfully and sediment deposition has almost completely covered the structures that were installed in 2016 and vegetation is now growing on top of them. If you need more information about the changes or if you would like to meet at the site to review the completed work, please let me know. It appears from your email below that these structures are currently being constructed. Additional Condition 1 of the 401 Water Quality Certification approval for the North Pacolet River Reach 4 project, issued September 4, 2014, to Polk County Soil and Water Conservation District (attached) states "This approval is for the purpose and design described in your application. The plans and specifications for this project are incorporated by reference as part of the Certification/Authorization. If you change your project, you must notify the Division and you may be required to submit a new application package with the appropriate fee. I don't have any records that indicate the modifications to the plan have been approved. Was the Division notified of these design changes and was any type of approval for the design modifications issued by the Division? Andrew W. Moore, P.G. The plans were originally prepared by Zan Price in 2014. Conditions have changed since then that required modification of the plans. This has required modifications to Zan's plans. Zans original plans were included to show the portions of the project on C-6.0 that were not revised. The more recently certified sheet C-6.0 which was signed by Tony Hauser shows revisions in Phase 3 of construction which are currently underway. We have not made revisions to Zan's plans in Phase 4 so his original plan is included to show what is proposed in that area. The Division has determined that your application is incomplete and cannot be processed. The application is on hold until the following items are addressed: 1. Please provide construction details for the Longitudinal Peaked Stone Toe (LPST), LPST Tie-back/keyway, and Stream Barb. Please clarify the design approach for approximate station 38+50 to 42+50. Two Sheet C-6.0 were provided with the Pre -Construction Notification, which appear to show two separate designs within the same area. Pursuant to Title 15A NCAC 02H .0502(c), the applicant shall furnish all of the above requested information for the proper consideration of the application. If all of the requested information is not received within 30 days, the Division will be unable to approve the application and it will be returned.
