Additional information will be required in order to complete the review process. Please provide the following information so that we may continue to review your project. Additional Information Requested: 1. Impact Justification The application states the purpose of the proposed work to be: "To provide additional usable land for the commercial development." The drawings provided, depict a commercial building and parking lot accessed from Setser Branch Road. The stream to be impacted bounds the property along the southern edge and does not appear to negatively affect development of the property as stated. Therefore, the impacts to waters do not seem to be necessary for the project to be completed. Please provide other information supporting the stream impacts. Project Drawings Your enclosed drawings lack sufficient detail for this office to process. You must include: Plan View o North Arrow o Landmark or road intersection that would provide positional detail o All graded areas, finished cut and fill slopes, and required setback (or buffer) areas o Existing and finish elevations o Fill and culvert alignment o Inlet / outlet protection o Scale (1 inch = 50 feet scale or larger) Cross Sectional View o MSL elevation o Culvert bedding detail o Finish fill elevation o Protective barriers and overlays o Any overlay construction detail (sub pavement / pavement) 3. Stormwater Management Plan According to the application, the project impervious surface is estimated to be 50%. Projects with imperviousness greater than 24% require a stormwater management plan be developed and approved. Please provide three (3) copies of stormwater management plans along with all supporting information pertaining to this project including the following: (i) Entire site development/layout plans delineating all drainage areas (ii) Design calculation sheets for all proposed BMP(s) sized for both on-site and off-site drainage (iii)BMP supplements for each proposed BMP and Required Items Check List (http://h2o.enr.state.nc.us/su/bmp forms.htm) along with all required items (iv)A Notarized Operation & Maintenance Agreement for each Stormwater management facility (v) Detailed construction drawing sheets shall include: a. Erosion and sediment control plans b. Installation and planting schedules for the proposed BMPs Appropriately scaled plan views (1":50' scale) and cross sectional details of all BMPs and associated components - inlet and outlet structures, forebay, respective zones and pertinent elevations. 4. Collective Impacts According to the application and Macon County land records, Mr. Vanderwoude is the registered agent of Big Bear of North Carolina, LLC-owner of the property under application [ PIN Nancy mernman 'February 26, 2009 Page 3 o1 A 6583.04-52-3432]-and owner of the adjoining property to the west [PIN 6583.04-52-1324] through which flows the stream proposed to be impacted. The proposal describes attaching new culvert to the existing culvert on Mr. Vanderwoude's property [6583.04-52-3432]. The DWQ must review projects for collective impacts. Please provide information about the installation of culvert on the property identified by PIN 6583.04-52-3432, including: 1. The dates of culvert installation 2. Property owner information at the time of culvert installation 3. Any permitting information related to culvert installation (i.e., US Army Corps of Engineers permitting documents) 4. The length of the installed culvert 5. As-built drawings of the existing culvert 5. Mitigation The application states: VIII. Mitigation 1. "There is no mitigation plan for this lot." VIII Mitigation 2. Amount of stream mitigation requested (linear feet): 150 Please indicate how mitigation requirements will be met. Please provide your response in writing to the addresses below prior to March 30, 2009 or 30 days from receipt.
