Please find attached the application form we spoke of this morning for requesting on-site determinations of "blue -line"
features. If you aren't the owner of the property, you'll also need to include some sort of agent authorization from the
owner that allows me/DWR on the property. I've attached a link to an example agent authorization form below. Also,
please make sure to include some sort of property location map (such as a tax parcel map), so that I can find the water
feature on the maps that we use for identifying those "blue -line" features.
If you have any questions, please let me know how I can be of help. Just so you're aware for planning purposes, the
turn -around time for these on-site determinations usually averages around 20-30 days, including the site visit. If you
would like to be present when the determination is done, I can schedule a time with you. If you don't need to be
present, then sometimes that gives me a little flexibility in making the site visit in route to other already planned site
visits, and shortens the turn -around time.
