Subject: Response to Notice of Deficiency
Ms. Chapman,
This letter is to address the issues that resulted in a Notice of Deficiency on the above
referenced project, as stated in the letter dated Mazch 2, 2018. The issues of the
inspection forms not being properly completed and maintained have been addressed and
corrected. The issues developed when the contractor's Level II Erosion Control
Supervisor did not correct the reports as requested by the NCDOT representative and Mr.
Gazcy Wazd. The contractor stated he would the correct them, so we gave the contractor
another opportunity to resolve the issue, before the NOD was issued. The Contractor's
higher level personnel responded when the NOD was issued.
Since this NOD was issued we have spoken to our project inspectors and reviewed the
proper completion of the reports. We have also addressed the contractors, both this
specific contractor and others, regarding the reports. In the future we aze prepazed to stop
work on projects that have these issues in order to get them resolved in an expeditious
manner.
