The project, is a 1.99 mile, three-lane divided facility on new location extending from US 158 to US 311 near Winston Salem, Forsyth County, North Carolina. Coordinates (in decimal degrees) for the site are 36.1500°N, -80.1695° W. The project area contains a portion of Frazier Creek, three (3) unnamed tributaries to Frazier Creek, Lowery Mill Creek, one (1) unnamed tributary to Lowery Mill Creek, and four (4) adjacent wefland areas in the Yadkin River Basin (8-Digit Cataloging Unit 03040101). Impacts authorized by the pernut dated August 17, 2017 include: 1) the permanent placement of fill material into 977 lineaz feet of jurisdictional stream channel (921 linear feet of permauent loss and 761inear feet of bank stabilization); 2) 0.88 acre of adjacent riparian wetlands (0.79 acre of permanent fill and 0.09 acre of inechanized clearing); and 3) the temporary placement of fill material into 1991inear feet of jurisdictional stream channel. Mitigation was implemented for the unavoidable unpacts by payment into the North Carolina Division of Mitigation Services. Also reference your letter to the U.S. Army Corps of Engineers (Corps) dated February 6, 2018 (received February 6, 2018), requesting modification of the permit for the construction of a temparary haul road which includes additional impacts to 0.13 acre of wetlands at Permit Site #12. The approxitnate area of nnpact will occur from the toe of slope to the right-of-way from Station 475+00 to 476+00. In addifion, the modification request also includes the downstream shift of the location for the crossing of Lowery Mill Creek at permit site #13. This shift will be maintained in the previously permitted bank stabilization limits. -Z- The Corps has completed the evaluation of your request and determined that it is appropriate and reasonable, and no public notice is required for this modification. Therefore, with the changes to the Special Conditions listed below the permit is modified as requested for a net increase of temporary impacts to wetlands of 0.13 acre. Project U-2579C now totals permanent nnpacts to 9771inear feet of jurisdictional stream cl�aunel (921 linear feet of permanent loss and 761inear feet of bank stabilization), 0.88 acre of adjacent riparian wetlands (0.79 acre of permanent fill and 0.09 acre of inechanized clearing), and the temporary placement of fill material into 1991inear feet of jurisdictional stream channel and 0.13 acre of jurisdictional riparian wetlands. As stated above, this permit modification is contingent upon the following changes to the Special Conditions: 1. CONSTRUCTION PLANS: All work authorized by this permit must be performed in strict compliance with the attached plans for U-2579C, Wetland Surface Water Impacts Permit, dated June 13, 2017, and those attaehed to the permit modification request dated February 6, 2018. Any modification to these plans must be approved bythe US Army Corps of Engineers (USACE) prior to implementation. 24. Temporary wetland fills must be removed in their entirety, returned to pre- construction elevations, and revegetated, per the restoration information submitted in the February 6, 2018 modification request. All other conditions of the permit, including the permit expiration date of December 31, 2022, remain in effect as written.
