Dear Mr. Thompson:
On March 10, 2009,the Division of Water Quality(DWQ)received your application dated March 9,
2009,to fill or otherwise impact 11 linear feet of perennial stream(138 linear feet of perennial stream
previously permitted), and 192 square feet of Zone 1 Neuse River basin protected riparian buffers and 87
square feet of Zone 2 Neuse River basin protected riparian buffers to extend a previously permitted
culvert at the site. The DWQ has determined that your application was incomplete and/or provided
inaccurate information as discussed below. The DWQ will require additional information in order to
process your application to impact protected wetlands and/or streams on the subject property. Therefore,
unless we receive five copies of the additional information requested below, we will place this project on
hold as incomplete until we receive this additional information. If we do not receive the requested
information,your project will be formally returned as incomplete. Please provide the following
information so that we may continue to review your project.
Additional Information Requested:
1. Please address the following issues with the proposed 54-foot level spreader:
a. Provide calculations to show that the velocity of flow from the dissipater pad will not
exceed two feet per second during the peak flow from the ten-year storm. The dissipater
pad may need to be enlarged to meet this criteria.
b. The distance between the base of the level spreader and the top of stream bank appears to
be less than the 75 feet that is stated in the Supplement Form.
c. The photo of the riparian buffer that is provided does not show any grassed areas. If you
are planning to establish grass within Zone 2 of the buffer to facilitate the performance of
the level spreader, then please provide planting and water specifications. Sod is the
preferred method for establishing grass down slope of a level spreader.
2. Please address the following issues with the proposed scour hole:
a. Provide sizing information for the scour hole that is based on Chapter 8 of the NC
Stormwater BMP Manual.

b. The scour hole should be re-oriented such that it is parallel with contour lines.
3. Please provide plan details of the proposed level spreader and scour hole on a full-sized plan
sheet at a scale of 1"=30' or larger.
Please submit this information within 30 calendar days of the date of this letter. If we do not receive this
requested information within 30 calendar days of the date of this letter,your project will be withdrawn and
you will need to reapply with a new application and a new fee.
This letter only addresses the application review and does not authorize any impacts to wetlands, waters or
protected buffers. Please be aware that any impacts requested within your application are not authorized(at
