I am writing this letter to respond to the NOV that our facility received on March 8, 2018.
First of all I would like to thank you for all your help and advice during our phone
conversation after your visit to our facility in Maiden. I know that we made several mistakes
during the months of November and December 2017 while Wayne Kelly, or ORC was out for
medical reasons. I take full responsibility for not assuring things were done properly during
that time period. I was familiar with waste water treatment sent to the City of Lincolnton but
not as familiar with our current waste water treatment facility. I had hired Allen Lynch to be
our backup ORC and relied on him to operate the facility during Wayne's absence.
I think our problems started when the below freezing temperatures started and when we had
to fill out the state reports without Wayne's help. We were doing the proper testing but were
not reporting properly.
To address the monitoring during the weeks of November 19, 2017 and December 24, 2017, 1
would have to say that we were confused about holiday monitoring procedures during
Wayne's absence. As you know I have hired Donald Burkey from the City of Lincolnton and he
is now our ORC. I am confident that he will know everything required and will train the

backup ORC and his helper to properly monitor the facility while he is here and if he has to be
away for any reason.
To address the failure to report the bypass that occurred when the overflow pipe froze and
broke, I would again have to fall back to our ORC not being present and that our backup ORC
thought that he had contained the bypass. I know now to report immediately any problems
as soon as they occur. I will require the ORC and the backup ORC to do this as mandatory to
continue their employment with South Fork Industries Inc. If for any reason they are not
available I will contact your office in the future.
To address the subject of failure to visit the facility during the week of December 2017, 1 will
again state that we did have someone to come to the facility but they were not the ORC or
the backup ORC as required in our permit. The helper did come to the facility but again that
did not meet our permit requirements. Our ORC was out due to health problems and our
backup did not properly understand the holiday requirements. We have addressed all these
situations with Donald and are requiring him to become very familiar with all requirements in
our permit.
To address the other deficiencies stated in your report I asked Donald to help me address the
concerns and violations listed.
RECORD KEEPING
His response was that we have hired a new ORC and he is properly training the backup ORC
and the operator about the proper sampling schedule and the frequency that each parameter
needs to be sampled. This should eliminate any future violations in this area.
LAB SECTION
The D.O. meter is being verified with the standardized chart for all future references.
The PH buffers are disposed of after each use and new ones for every testing event.
The PH samples are time stamped and documented pickup and analyzation times are being
recorded to insure testing done in required 15 minute time limits.

We ordered certified thermometers that should arrive this week to insure proper
temperatures for our samplers and the refrigeration storage. Temperature readings will be
recorded daily.
OPERATIONS AND MAINTENANCE
The amount of knitting oil in aeration basin has been significantly reduced and we are
working to completely eliminate it ASAP .We are no longer allowing any to be added to the
waste water facility.
The above ground storage tank has been relocated to an area that does not drain into the
waste water treatment facility.
Vertical crack in the Peroxide and Salt storage area has been repaired and we are getting
quotes to resurface the inside of the containment area with a chemical resistant epoxy to
insure its containment properties are intact.
I do not intend on making any excuses for the occurrences that we had over the past few
months but I do what you to please take into consideration that we are new to this type of
waste water treatment facility and were heavily relying on Wayne Kelly as our ORC to
operate the facility. I know that things were properly operated and procedures followed
during his tenure with us. I also need for you to know that we will do everything possible to
acquire the proper personnel to insure we meet or exceed all requirements of our permit. We
have two employees that have completed the first required school time and will be testing
soon to become level one operators and hopefully will reach level 5 in the future with Donald
Burkey's leaderships.
Our company purchased this location and we have completed the move of our entire location
from the Lincolnton NC facility to this Maiden NC facility during the past 12 months. During
that time I have tried to insure that we acquired the proper permitting and the proper
personnel to help meet the requirements of all permits. We are currently working to get all
needed information to all state officials for permitting. I would again thank you for all your
help and please contact me if any further information is required.
