Describe the existing conditions on the site and the general land use in the vicinity of the project at the time of this application: Dotey's Creek (a non-navigatable stream) flows through two existing 36" X 52' CMPs and one 40" X 52' elliptical CMP located under Red Oak Street just upstream of the drained Elizabeth Lake in Jacksonville, NC. Several single family home lots border the stream however there is a 25 foot vegetated shrub and forested buffer downstream of the culvert. 3b. List the total estimated acreage of all existing wetlands on the property: 0.03 acres 3c. List the total estimated linear feet of all existing streams (intermittent and perennial) on the property: 52 feet of intermittent stream channel. 3d. Explain the purpose of the proposed project: Replace three deteriorating culverts (CMP) and headwalls for safety reasons. Additionally, the 3 culverts will be upgraded to three 42" X 52' culverts to accommodate future development in watershed. 3e. Describe the overall project in detail, including the type of equipment to be used: Replace two deteriorating headwalls, two 36" X 52' culverts and one 40" X 52' elliptical CMPs with new headwalls and three 42" X 52' CMPs. In conjunction with the culvert replacement the City will be replacing the existing clay sewer pipe, cast iron water line and plastic force main that lies under the road and above the culverts at the same vertical and horizontal slope. Construction will entail cutting out the section of sewer, water and force main and replacing with restrained joint ductile iron pipe for safety and to minimize the chance of spills into the creek. Construction also entails building a pump around with two impervious dikes. The pump around will discharge into a 4'x 6'x 12' rumble riprap pad to stabilize outlet for discharge of clean water. A second pump will be set up for dewatering the excavated construction area and will discharge first into a silt bag then through the 4'x 6'x 12' rumble riprap pad for discharge of clean water. An additional permanent rumble riprap pad 12'x 17'x 18' with filter fabric will be installed downstream of the culverts to prevent scouring. A trackhoe and dump truck will be used to to replace deteriorating culverts. 4. Jurisdictional Determinations 4a.
