Dear Mr. Huntley:
This letter is to provide you copies of the subject Civil Penalty Assessment documents as
mentioned during our phone conversation, at 11:03 a.m. on March 23, 2018, with Mr. Roberto
Scheller and myself. Specifically, enclosed are copies of Enforcement Case No. LV -2017-0309
for violations that occurred during the month of July 2017 with an outstanding civil penalty in the
amount of $703.62 and Enforcement Case No. LV -2017-0325 for violations that occurred during
the month of February 2017 with an outstanding civil penalty of $453.62. The total outstanding
civil penalty balance is $1,157.24.
Please note that the 30 -day response time period has been exceeded, therefore it is necessary
to contact this Office to procure a settlement in the subject cases.
