(1) Applicability:
n The project is new or a major modification of an existing permit
All off the MDC will be met and the project is not proposing an alternative design
x (a) The project is not claiming an exemption from the MDC
na (b) Modifications to existing projects will meet the 2017 rules, including MDC
x (c) The project is not out of compliance with an existing stormwater permit

x Using current version of the application
2 hard copies and 1 electronic copy of the following:
x (a) Completed and signed Fast -Track Application, including the following:
x (i) Project name - current and previous (if applicable)
x (ii) Project location
(iii) Previous stormwater project number (if applicable)
x (iv) Applicability of other state and federal permits
x (v) Applicant name, address, and contact info
x (vi) Owner name, address, and contact info

(vii) Certification of financially responsible owner
(b) When the applicant is a corporation or LLC:
e (i) Documentation that applicant is active and in good standing with NCSOS
(ii) Documentation from NCSOS or other official documentation supporting the titles / positions held by person(s) signing the
application pursuant to Rule .1040(1)

LLC: by manager or company official as defined in G.S. 57D
(b) Partnership: by a general partner or managing partner
