Beauregard, NCDOT NEU (present) Betsy Cox, NCDOT Structures (present) The 4B meeting held on 6/18/08 began with Randy Henegar providing an overview of the project and handing out plan sheets with delineated streams and wetlands. The areas on the project where a pipe and slope failure has occurred will be permitted with a Nationwide #3. The stormwater is being treated with grassed swales. A sheet by sheet review of the project was conducted. Sheets 4-12: Jurisdictional streams and wetlands were identified. There are no anticipated impacts at these sites and there were no further comments. Sheet 13: There is a slope and pipe failure right of Sta. 155+00. This will be repaired on this project and permitted with a Nationwide #3. The work on this project is in the median unless work on the outside is required to repair slope and/or pipe failures. Hydraulics has not looked at all pipes yet, so there may be additional areas needing repair. Sheet 14-20: Jurisdictional streams and wetlands were identified. There are no anticipated impacts at these sites and there were no further comments. Sheet 21: Pipe and slope failure occurring outside of ramp left of Sta. 265+50. This will be repaired on this project. There will be no stream impacts or work outside of the Right-of-Way. A jurisdictional stream is 80 feet from the R/W. There are no anticipated buffer impacts at this site. Sheets 22-25: Jurisdictional streams and wetlands were identified. There are no anticipated impacts at these sites and there were no further comments. Additional Comments: The only impacts anticipated on this project are at sites where pipe and slope failures are occurring and need to be repaired. There most likely will be buffer impacts at these sites. Underdrain will be installed at the request of the Division on the entire project in the median. Hydraulics proposed the question as to if a 4C meeting would be necessary? The consensus was that it would not be necessary. NCDWQ indicated they would like a set of drawings to review and provide comments. mts
