In order to facilitate the review and processing of land application program annual reports, the
Division is requesting your cooperation in adjusting the order in which the required forms are
submitted. Beginning on the next annual report submission (2008 annual report), the Division is
requesting that all annual distribution & marketing reports be submitted in accordance with the
document list provided below. As a reminder, the deadline for the 2008 annual report
submission is on March 1, 2009.
Annual report packages should be organized and bound (stapled, or small binder, but please no
three-ring binder unless necessary) in the following order prior to submission:
1. Cover Letter
2. Class A Distribution & Marketing/Surface Disposal Certification and Summary Forms:
DMSDF
3. Annual Residuals Sampling Summary Forms: RSSF
4. Residuals Analysis Lab Sheets
5. TCLP Lab Sheets
6. Annual Pathogen & Vector Attraction Reduction Forms: PVRF
7. Pathogen & Vector Attraction Reduction Lab Sheets/Records

Please note that only the most recent Division forms, or forms prepared by others and previously
approved by the Division may be used for submittal of the annual distribution & marketing
report. The Division must approve other forms before they are submitted as part of the annual
report, or the annual report may be returned as incomplete. Contact the Land Application Unit
for more information on adaptations of existing forms.
As a reminder for the land application program permitted under 2T regulations, please make sure
that the most recent "02T Land Application Report Forms" are being used for your program.
