your permit. It is required of any person holding a permit for any time during the annual fee period, regardles f the facility's
operating status. Failure to pay the fee by the due date will subject the permit to revocation. Operating wi ut a valid
permit is a violation and is subject to a $10,000 per day fine. If the permit is revoked and you later deci a permit is
needed, you must reapply, with the understanding the permit request may be denied due to changes? in environmental,

This letter, along with the enclosed completed application and supporting documentation,
is sent to request renewal of an existing Pump and Haul Permit at the above -referenced facility.
As noted briefly in the application, the Onslow Water and Sewer Authority (ONWASA)
has been working steadily over the past two years to rehabilitate the existing wastewater
treatment plant serving the Summerhouse at Everett Bay residential development. This facility
was constructed by the original site developer in 2008 and then transferred to ONWASA in
2009. Despite its relatively recent construction, significant work has been needed to ensure plant
systems operate correctly and are compliant with State requirements. The remaining items of
work necessary to achieve formal approval to begin operation are currently on track to be
completed by the end of March 2017. ONWASA is therefore requesting permission to renew the
existing permit, which will expire February 15, 2017, in order to maintain a permitted pump and
haul volume of 8,000 gpd that represents influent from the more than 20 occupied homes and
clubhouse within the Summerhouse development.
In accordance with previous pump and haul permits, the proposed operation will consist
of pumping from existing Summerhouse wastewater pumping stations No. 2, 3, and 4, with
discharge at one of two locations depending on available capacity at the time. The primary
discharge location will be an existing manhole adjacent to the intersection of Jenkins Street and
Crawford Street in the Town of Holly Ridge, NC. From that point, the wastewater will be
conveyed to ONWASA's Holly Ridge Wastewater Treatment Facility. In the event of inclement
weather and resulting increased flow at the Holly Ridge site, the secondary discharge location
will be an existing manhole on Rolling Ridge Drive in Jacksonville, NC, which is upstream of
the nearby Hunters Creek Pump Station. The Hunters Creek Pump Station conveys wastewater
to the Piney Green Pump Station, which then directs the flow to French Creek Wastewater
Treatment Facility on Camp Lejeune for treatment. ONWASA has sufficient wastewater
allocation at the French Creek Wastewater Treatment Facility to accommodate the additional
flow.

The following information, as stipulated on the application form, is enclosed for your

• A check in the amount of $810.00 made payable to NCDENR for the fee required as the
flow being requested not to exceed 9,999 gpd.
• Letter of flow acceptance from the ORC of the Holly Ridge Wastewater Treatment
Facility
• Letter of flow acceptance from the ONWASA Collection System ORC
• Copy of Addendum III of the Wastewater Sales Agreement between Camp Lejeune and
ONWASA confirming that ONWASA has 3.5 MGD of allocated wastewater capacity at
the French Creek treatment facility
• Letter from the hauler stating their capability and willingness to perform the pumping and
hauling
• Two (2) sets of Plans (Maps showing the proposed pump and haul routes and Record
Drawing of Summerhouse Pump Station Nos. 2, 3, and 4)
• Two (2) copies of the technical specifications
