A. General Program information: Enclosed is a copy of the Pretreatment Program Info
sheet provided by the PERCS unit. PPS,form, SNCR form, IDSF forms, and AT. As shown
Beocare is SIU permit#008 and has 1 discharge pipe. Lenoir Mirror is SIU#007 with 1
discharge pipe. These two industries discharge to the collection system that flows to
the Gunpowder Creek WWTP. The analysis results of the permitted parameters do no
show over allocation as per the AT provided in the HWA; permit limits should remain
the same.
B. General permit information for Gunpowder Creek: No new industries were added as
permitted SIU's in 2017.The permit for both Beocare and Lenoir Mirror expired on
December 31, 2017. Neither industry sent the required permit renewal application in to
the City of Lenoir the 180 days prior to it expiring. I have been working with Deborah
Gore to get the permit process completed and have gotten most of it in. However, they
do have a new permit with dates of January 31, 2018-December 31, 2022 listed on it.
No permit limits changed. We are now awaiting permit approval for them of which I
need to change a few things, and resubmit to the PERCS unit.
1. Beocare: is a medical textiles facility discharging domestic waste and process
wastewaters to the collection system. The WWTP monitored the flow, analyzed
for BOD,TSS, Cr, Cu, and Zn, with permit limits of BOD-400mg/I, TSS-400 mg/I,
Cr- 0.400 mg/I, Cu-0.300 mg/I, and Zn-0.500 mg/I
2. Lenoir Mirror: is a metal finishing facility discharging domestic waste, process
wastewaters and wash waters to the collection system. The WWTP monitored
the flow, and analyzed for BOD,TSS,As, Cd, Cr, Cu, CN, Pb, Ag, Zn, and Total
Metals which is the sum of Cu, Ni, Cr, and Zn, with permit limits of TSS-500 mg/I,
As-0.060 mg/I, Cd-0.010 mg/I, Cr, 0.100 mg/I, Cu- 1.200 mg/I, CN-0.020 mg/I,
Pb-0.075 mg/I,Ag-0.075 mg/I, Zn-0.600 mg/I and Total Metals- 2.12 mg/I.
Total Toxic Organics (TTO) shall also be monitored, but a certification statement
may also be used for TTO's.

IU INFORMATION
A. IU's: SIU—permit 007 Lenoir Mirror, and SIU—permit#008 Beocare
B. IU's in SNC information: There were some sample collection issues for certain
parameters for Beocare and Lenoir Mirror during the 2017 year.
I recently learned that if an industry has a permit limit for a POC,that it is to be analyzed
3 times per year, and Becocare and Lenoir Mirror both have some POC's listed on their
permits with limits. I will address that beginning this year to add the additional
collection of those parameters to meet the requirement.
I am relatively new to the pretreatment program for the City of Lenoir.As well as being
newly appointed as Superintendent of the facilities, I have also inherited the
pretreatment coordinator position. Our sample technician is also relatively new to the
position as well as our lab supervisor.
With all that being stated, I wanted to express our willingness and concerns to get
everything right, and continue to provide quality data and information for the
Pretreatment program.
1. Beocare: Our sample technician did not collect a sample for the 2nd 6 month
period for Cr, Cu, or Zn. It was not found by the lab supervisor or me until I
began working on the PAR.
a. Addressing Cr- it was in regular/TRC violation during the 1st 6 month
period with a result of 0.525 mg/I (limit is 0.400 mg/I TRC 0.480 mg/I), however
during the first 6 month period there were a total of 7 samples collected
with only the one sample being in violation,therefore they were not
considered SNC for the first 6 months, and no sample was collected for
the 2nd 6 month period.
b. Addressing Cu-it was not in violation during the 1st 6 month period.
There were a total of 6 samples collected and analyzed for that 1St 6
month period and none of those results were out of compliance, and no
sample was collected for the 2nd 6 month period as per permit
requirment
c. Addressing Zn- it was in regular violation during the 1St 6 months period
with a result of 0.575 mg/I (limit is 0.500 mg/I,TRC 0.600 mg/I), however during
the first 6 month period there were a total of 7 samples collected with
only the one sample being in violation,therefore they were not
considered SNC for the first 6 months, and no sample was collected for
the 2nd 6 month period.

2. Lenoir Mirror-Our sample technician did not collected CN during the ft 6 month
period. There was not a CN sample collected for the 2nd 6 month period as per
the permit requirement(1 time per 6 months). Lenoir Mirror sample collection was
really a mess, and hopefully this explanation will be clear to understand.
Cd, Cr, Cu, Ni, Zn, and Total Metals have a permit requirement of quarterly(which
would be at least 2 samples for each 6 month period) sample analysis. Because of the Cu,
Cr, Ni, and Zn missing from the 4th quarter,the Total Metals was SNC
a. Addressing CN-The Cn was missed during the ft 6 month period,
however it was collected for the 2nd 6 month period and the result was
<0.05 mg/I
b. Addressing Cr-The Cr was collected 1st, 2nd, and 3rd quarters. The 1St, and
2nd quarters are included in the ft 6 month period and there were a total
of 7 samples collected during the ft 6 month period none of the samples
being in violation and thus were not considered SNC. The 3rd quarter
analysis collected was not in violation, but the 4th quarter was missing.
c. Addressing Cu-The Cu was collected 1St,and 2nd quarters.The ft 6 month
period (includes 1st,and 2nd quarters)there were a total of 7 samples collected
during the ft 6 month period with only one sample that was a
regular/TRC violation with a result of 1.88 mg/I (limit is 1.2mg/I,TRC is 1.44
mg/I), but were not SNC for the ft 6 month period.
The 2nd 6 months Cu was only collected once which was during the 3rd
quarter. It was not collected for the permit required 4th quarter. The one
result that was obtained was 1.47 mg/I which was a regular/TRC
violation, and because no other samples were collected they were

out.
d. Addressing Ni-The Ni was collected ft,and 2nd quarters.The ft 6 month
period (includes 15t,and 2nd quarters)there were a total of 7 samples collected
during the ft 6 month period and none were in violation.They were not
considered SNC for the 1st 6 month period.
The 2nd 6 month period, NI was only collected during the 3rd quarter
which was a permit violation requirement of quarterly. The one result
was< 0.001 mg/I.

e. Addressing Zn-The Zn was collected 15t,and 2nd quarters.The 1st 6 month
period (includes 1St,and 2nd quarters)there were a total of 7 samples collected
during the 15t 6 month period with only one sample that was a
regular/TRC violation with a result of 2.01 mg/I (limit is 0.6mg/I,TRC is 0.72
mg/I), but were not SNC for the 1st 6 month period
The 2nd 6 months Zn was only collected once which was during the 3rd
quarter. It was not collected for the permit required 4th quarter. The one
result that was obtained was 2.71 mg/I (limit is 0.6,TRC is 0.72 mg/I)which
was a regular/TRC violation, and because no other samples were
collected they were considered SNC for the 2nd 6 month period. A
NOV did not get sent out.
f. Addressing Total Metals-The Total Metals was calculated (sum of Cu,Cr, Ni,
and Zn) 1st, and 2nd quarters.The 1st 6 month period (includes 15t,and 2nd
quarters)there were a total of 7 samples collected during the 1st 6 month
period with one sample that was a regular violation and 1 sample that
was regular/TRC violation (limit is s s mg/I TRC is 2 16 mg/I)with a resu It of 2.01
mg/I (limit is 0.6mg/I,TRC is 0.72 mg/I),but were not SNC for the 1st 6 month
period
The results of the Cu, Cr, Ni, and Zn for that one analysis collected during
the 3rd quarter were (Cu)1.47 mg/I, (Cr)0.006 mg/I, (Ni)<0.001 mg/I, and
(Zn)1.24 mg/I for a result of 2.71 mg/I for Total Metals for the 2nd 6 '
month period. They were considered SNC for the 2nd 6 month period.
A NOV did not get sent out.
From all this explanation I will sum it up for Lenoir Mirror as three (3)SNC
violations for the 2nd 6 month period for Cu, Zn, and Total Metals. There were
permit requirement violations of CN not being collected the 1st 6 month period,
and Cd, Cr, Cu, Ni, Zn, and Total Metals permit requirement violation of quarterly
sampling which in my opinion had we sampled them at least once during that 4th
quarter may have gotten them out of SNC violations.
To sum up Beocare,they did not have any SNC violations, only the permit
required violation of 2nd 6 month period sample collections.
