The Onslow Water & Sewer Authority (ONWASA) is permitted to operate a 400,000 gpd
Nonconjunctive Reclaimed Water Utilization System with infiltration disposal at the Summerhouse
S/D. ONWASA has requested to perform pumping and hauling from Summerhouse wastewater
pumping stations 2, 3, &4 to a. manhole tributary to the influent pumping station at the Hunters
Creek WWTP, tributary to the MCB Camp Lejeune Advanced Wastewater Plant as well as to a
manhole tributary the Holly Ridge WWTP for a permitted pump & haul volume of up to 8000 gpd
of raw. domestic wastewater. Wastewater sent to an upgradient manhole tributary to the Hunters
Creek influent pumping station is to be processed at the MCB Camp Lejeune Advanced
Wastewater Plant in accordance the conditions of Permit No. NCO063 029 per the agreement
between ONWASA and the MCB Camp Lejeune, and' wastewater sent to an upgradient manhole of
the Holly Ridge WWTP is to be processed in accordance with Permit No. WQ0019907.

In accordance with your application received January 23, 2015 we are forwarding herewith Permit No.
WQ0037575, dated February 1, 2015 to the Onslow Water &Sewer Authority.. This permit is provided for the
temporary operation of pump and haul activities transporting up to 8000 gpd of raw domestic wastewater from the
Summerhouse sewer collection system (Pumping Stations #2,#3;), tributary to the Summerhouse WWTP
(Permit No. WQ0029945), to a sewer collection system manhole located immediately upgradient of the Town of
Holly Ridge WWTP or a sewer collection manhole located upgradient from the Hunters Creek pumping station
tributary to the MCB Camp Lejeune Advanced WWTP for ultimate treatment and disposal in accordance with
either Permit No. WQ0019907 (Holly Ridge WWTP) or Permit No. NC0063029 (MCB Camp Lejeune Advanced
WWTP) at volumes that will not exceed the receiving plant permitted limitations. This permit supersedes Permit
No. WQ0036955.
Thisru ermit shall be effective from Feb1� 2015 until February 1, 2016 and shall be subject to the

conditions and limitations as specified therein. Please pay particular attention to the monitoring requirements in
thisyermit. Failure to establish an adequate system for collecting and maintaining the required operational

information will constitute noncompliance with the permit. Also, please note Condition No. 8 in the permit, which
states that the pump and haul activity will. cease upon the expiration date.
If any parts, requirements, or limitations contained in this permit are unacceptable to you, you have the right
request tto request an adjudicatory hearing upon written within 30 days following receipt of this permit. This reques
must be in the form of a written petition, conforming to Chapter 150B of North Carolina General Statutes, and filed
with .the Office of Administrative Hearings, P.O. Drawer 27447, Raleigh, NC 27611-7447. Unless such demands
are made, this permit shall be final and binding.

In accordance with the provisions of Article 21 of Chapter 143, General Statutes
of North Carolina as amended, and other. applicable Laws, Rules, and Regulations:
PERMISSION IS HEREBY GRANTED TO

for the
temporary operation of a pump and haul activity for transporting up to 8000 gpd of raw domestic
wastewater from the Summerhouse sewer collection system (Pumping Stations #2,#3,& #4),
tributary to the Summerhouse WWTP (Permit No.WQ0029945) located in Onslow County to a
manhole tributary to the Holly Ridge WWTP (Permit No. WQ0019907) in Onslow County or to a
manhole tributary to the Hunters Creek influent pumping station, tributary to the Marine Corps
Base Advanced WWTP (Permit No. NC0063029) with no discharge of wastes directly to the
surface waters, pursuant to the application submitted to the Department of Environment, and
Natural Resources and considered a part of this permit.
This permit shall be effective from the date of issuance until February 1, 2016 and shall be subject
to the following specified conditions and limitations:
1. This permit shall become voidable unless the subject pump and haul activities are carried out
in a manner which has been approved by this Division.
2. This permit is effective only with respect to the nature and volume of wastes described in the
application and other supporting data..
3. The facilities shall be properly maintained and operated at all times.

4. Thisp ermit is not transferable. In the event there is a desire for the facilities to change
ownership, or there is a name change of the Permittee, a formal permit request must be
submitted to the Division of Water Resources accompanied by an application fee,
documentation from the parties involved, and other supporting materials as may be appropriate.
The approval of this request ,will be considered on its merits and may or may not be approved.
5. No type of wastewater other than raw domestic waste from the Summerhouse Pumping Stations
#25#35 #4, shall be included in the pump and haul activities.
6. This permit shall become voidable unless the agreement between ONWASA-Holly
Ridge WWTP and the hauler (ONWASA- Collections & Distribution) and the agreement
between ONWASA and the Maine Corps Base Camp Lejeune are in full force and effect.
7. In the event that the facilities fail to perform satisfactorily, including the creation of nuisance
conditions, the Permittee shall cease operation of all pump and haul activities, and take such
immediate corrective action as may be required by this Division.
8. The pump and haul activity shall cease upon the expiration date of this permit.
9. The raw domestic wastewater collected from Summerhouse Pumping Stations #2,#3, #4 shall
be transferred to the upgradient manhole of the Holly Ridge WWTP for ultimate treatment and
disposal in accordance with conditions of Permit WQ0019907 or raw domestic wastewater
collected from Summerhouse Pumping Stations #2,#3, and #4 shall be transferred to a manhole
tributary to the influent pumping station at Hunters Creek WWTP for ultimate treatment and
disposal by the Advanced Wastewater Treatment facility at the MCB Camp Lejeune in
accordance with conditions of Permit No. NCO063 029 issued by the Division of Water
Resources. Such transfers shall be accomplished on Mondays and Fridays during regular daily
wastewater plant operating hours.
10. The Permittee (ONWASA) is liable for any damages caused by a spill or failure of the pump
and haul operations.
11. Adequate inspection, maintenance, and cleaning shall be provided by the Permittee or their
designated system operator, to insure proper operation of the subject facilities.
12. The Permittee (ONWASA) shall inspect the wastewater collection facilities and wastewater
plant receiving facilities to prevent malfunctions, deterioration, operator errors, and discharges

which may cause or lead to the release of wastes to the environment, a threat to human health,
.or a nuisance. The Permittee shall keep an inspection log or summary including at least the date
and time of inspection, observations made, and any maintenance, repairs, or corrective actions
taken by the Permittee. This log of inspections shall be maintained by the Permittee for as long

as the pump and haul activities are being conducted and shall be made available upon request to
the Division of 'Water Resources.
13. Any duly authorized officer, employee, or representative of the Division of Water Resources
may, upon presentation of credentials, enter and inspect any property, premises or place on
or related to the wastewater collection facilities at any reasonable time for the purpose of
determining compliance with this permit and may inspect or copy any records that must be
kept under the terms and conditions of this permit, and may obtain samples.
14. An accurate record of the pump and haul activities must be maintained by the Permittee
indicating:
date wastewater is removed from the facility.
name of facility from which wastewater is removed.
name of facility receiving wastewater.
volume of wastewater received at each wastewater treatment receiving facility.
status of permanent disposal option (Operation of Summerhouse WWTP)
These records shall be submitted to the Wilmington Regional Office of the Division of
Water Resources located at 127 Cardinal Drive Extension, Wilmington, North Carolina, 28405
on or before the fifteenth (15th) day of the following month.
15. Failure to abide by the conditions and limitations contained in this permit may subject the
Permittee to an enforcement action by the Division of Water Resources in accordance with
North Carolina General Statute 143-215.6A.
16. The issuance of this permit does not preclude the Permittee from complying with any and all
statutes, rules, regulations, or ordinances, which may be imposed by other government
agencies (local, state, and federal), which have jurisdiction.
17. Noncompliance Notification:
The Permittee shall report by telephone to the Wilmington Regional Office at telephone
no.(910) 796-7215 as soon as possible, but in no case more than 24 hours or on the next
working y da following the occurrence or first knowledge of the occurrence of any of the
following:
a. Any failure of equipment, storage or transportation facilities resulting in a by-pass directly
or indirectly to waters or lands of the State.

Persons reporting such occurrences by telephone shall also file a written report within 15
days following first knowledge of the occurrence. This report should be directed to
Morella S. King Ph.D, P.E., Environmental Program Supervisor III, NCDENR, 127
Cardinal Drive Extension, Wilmington, North Carolina 28405 and must outline the actions
taken to clean up the wastewater and ensure that the problem does not recur.
