In accordance with your application received August 24, 2012 we are forwarding herewith Permit No.
WQ0036136, dated September 19, 2012 to the Onslow Water & Sewer Authority, and is provided for the
temporary operation of pump and haul activities transporting up to 4100 gpd of raw domestic wastewater from the
Summerhouse sewer collection system Pumping Station #4, tributary to the Summerhouse WWTP (Permit
WQ0029945), to a sewer collection system manhole located immediately upstream of the Town of Holly Ridge
WWTP for ultimate treatment and disposal in accordance with Permit No. WQ0019907 and at volumes that v
not exceed the receiving plant's permitted limitations.
This permit shall be effective from September 19, 2012 until March 19, 2013 and shall be subject to the
conditions and limitations as specified therein. Please pay particular attention to the monitoring requirements in
this permit. Failure to establish an adequate system for collecting and maintaining the required operational
information will constitute noncompliance with the permit. Also, please note Condition No. 8 in the permit, which
states that the pump and haul activity will cease upon the expiration date.
If any parts, requirements, or limitations contained in this permit are unacceptable to you, you have the right
to request an adjudicatory hearing upon written request within 30 days following receipt of this permit. This request
must be in the form of a written petition, conforming to Chapter 150B of North Carolina General Statutes, and filed
with the Office of Administrative Hearings, P.O. Drawer 27447, Raleigh, NC 27611-7447. Unless such demands
are made, this permit shall be final and binding.

This permit shall be effective from the date of issuance until March 19, 2013 and shall be subject to
the following specified conditions and limitations:
1. This permit shall become voidable unless the subject pump and haul activities are carried out
in a manner which has been approved by this Division.
2. This permit is effective ' only with respect to the nature and volume of wastes described in the

from the parties involved, and other supporting materials as may be appropriate. The approval of
this request will be considered on its merits and may or may not be approved.
5. No type of wastewater other than raw domestic waste from the Summerhouse Pumping Station
#4, located in Onslow County, shall be included in the pump and haul activities.
6. This permit shall become voidable unless the agreements between ONWASA-Holly &haul
Ridge WWTP and the hauler (ONWASA- Collections & Distribution) relating top p
transfer receiving times and the receiving point are in full force and effect.
7. In the event that the facilities fail to perform satisfactorily, including the creation of nuisance
conditions, the Permittee shall cease operation of all pump and haul activities, and take
immediate corrective action as may be required by this Division.
8. The pump and haul activity shall cease upon the expiration date of this permit.
9. The raw domestic wastewater collected from Summerhouse Pumping Station #4 shall be
transferred to the upgradient manhole of the Holly Ridge WWTP for ultimate treatment and
disposal in accordance with conditions of Permit WQ0019907 issued by the Division of Water
Quality. Such transfers shall be accomplished on Mondays and Fridays during regular daily
wastewater plant operating hours.
10. The Permittee (ONWASA) is liable for any damages caused by a spill or failure of the pump
and haul operations.
11. Adequate inspection, maintenance, and cleaning shall be provided by the Permittee or their
designated system operator to insure proper operation of the subject facilities.
12. The Permittee (ONWASA) shall inspect the wastewater collection facilities and wastewater
plant receiving facilities to prevent malfunctions, deterioration, operator errors, and discharges
which may cause or lead to the release of wastes to the environment, a threat to human heath,
or a nuisance. The Permittee shall keep an inspection log or summary including at least the date
and time of inspection, observations made, and any maintenance, repairs, or corrective actions
taken by the Permittee. This log of inspections shall be maintained by the Permittee for as long
as the pump and haul activities are being conducted and shall be made available upon request to
the Division of Water Quality.

records shall be submitted to the Wilmington Regional Office of the Division of
Water Quality, Aquifer Protection Section located at 127 Cardinal Drive Extension,
Wilmington, North Carolina, 28405 on or before the fifteenth (15th) day of the following
month.
15. Failure to abide b the conditions and limitations contained in this permit may subject the
Permittee to an enforcement action by the Division of Water Quality in accordance with
North Carolina General Statute 143-215.6A.
16. The issuance of this permit does not preclude the Permittee from complying with any and all
statutes, rules, regulations, or ordinances, which may be imposed by other government
agencies (local, state, and federal), which have jurisdiction.

Onslow County
The Onslow Water & Sewer Authority (ONWASA) is permitted to operate a 400,000 gpd
Nonconjunctive Reclaimed Water Utilization System with infiltration disposal at the Summerhouse
S/D and has requested a pump and haul permit to send up to 4100 gpd of raw domestic wastewater
from the Summerhouse Pumping Station #4 to the Town of Holly Ridge wastewater treatment plant
under the control of ONWASA in lieu of beginning operations of the of the Summerhouse WWTP
(Permit No. WQ0029945). The Permittee indicates that an influent flow of 0.03 MGD is needed to
begin operations under Permit No. WQ0029945. There is currently 5 homes built in the
Summerhouse S/D and the club house/pool is in operation for the few homeowners.
The raw domestic wastewater will be removed from the Summerhouse Pumping Station #4 and
transferred in a leak -proof tanker by ONWASA to a sewer collection system manhole immediately
upgradient of the Holly Ridge WWTP (Permit No. WQ0019907) for ultimate treatment and
disposal. Pump and haul volumes are to be received at rates that will not exceed the influent
capacity of the receiving wastewater treatment plant.
