drainage area was assumed impervious execpt for pond slopes and drainage area to storm drain inlet 211 (223,889 sf total).

Shelf is planted at 50 plants per 200 sf (2,852 plants.) Species: Lance-leaf Coreopis, Purple Coneflower, Black-eyed Susan
and Scarlet Rose Mallow.

Attached you will find the following as requested:
1. SSW Supplement EZ Form. Wilbur Christmas is unavailable today to sign and seal the form, so I have signed in
his absence. I have been involved in the design of the pond throughout the process and am comfortable signing
the form. If you would like, Wilbur can sign and form and resubmit when he returns next week so that the
signature and seal match the plans. Please let me know if this is necessary.
2. The O&M agreement which has been signed by the Owner.
3. The SWM Maintenance Access easement is highlighted in green on the exhibit. We just received this yesterday
and were not able to get it on the drawing for submittal today. We will update the plans showing the easement
and submit early next week. Please accept this exhibit in the interim.

Neal,
I finished my review on the stomwater management plan submitted for the subject project and have only a few
comments below:
1. Please complete and provide stormwater supplement form for the proposed wet pond. The "Supplement EZ
form" can be downloaded at the website https://deg.nc.gov/about/divisions/energy-mineral-land-
resources/energy-mineral-land-permit-guidance/stormwater-bmp-manual.
2. Please complete and provide O&M form for the proposed wet pond. The "O&M EZ form" can be downloaded
from the same website link provided above.
3. Please modify one of the engineering drawings to include delineation of the maintenance access (min 10'
required and extend to the nearest public right-of-way) and easements.
Please send the requested information directly to me (via email is ok) within 15 days. Feel free to let me know if you
have any questions. Thank you!
