Dear Mr. Church:
On February 20, 2018, Paul DiMatteo and Jim Gonsiewski of the Division of Water Resources
("Division"), Winston-Salem Regional Office conducted a Compliance Evaluation Inspection of the above
referenced wastewater treatment system. Your presence and cooperation was greatly appreciated.This type
of inspection consists of two basic parts: an on-site inspection of the facility and a review of facility files and
self-monitoring data. Observations from each area of the inspection are summarized below.
Site Review
1. Permit
The current permit became effective on February 1, 2018 and expires on March 31, 2022.
A copy of the current permit was available for review. Permitted components include an automatic
bar screen, oxidation ditch, secondary clarifiers,tertiary filters, chlorine disinfection, and
dechlorination with final disposal to Naked Creek, Class C+waters in the New River Basin.
2. Facility Site Review
The facility was as described in the permit, and is operated 7 days per week.The facility is
permitted to discharge up to 0.6 million gallons per day(MGD) of treated wastewater;typical daily
discharges are about 0.22 MGD. All components appeared to be working properly. An emergency
generator was available that Mr. Church said could power the entire plant if necessary.
3. Operation and Maintenance
Tim Church is the primary Operator in Responsible Charge (ORC), and Joel Walters is the backup-
ORC. All operators are properly designated and hold adequate certifications. Mr. Church said he is
typically on site 7 days per week. The facility was very clean at the time of the inspection and
appeared to be well operated and managed.

4. Flow Measurement -
Effluent flow is measured using an ultrasonic device over a 90 degree V-notch sharp crested weir. A
Chessel type flow recording device is used with a seven day circular chart. The flow meter's
calibration was last checked by the Town of Jefferson on November 13, 2017.
5. Effluent/Receiving Waters
The effluent is discharged to Naked Creek in the New River basin. A path to the outfall was well
maintained and the effluent appeared clear with no visible solids or foam, and no visible impact to
the receiving stream.
Documentation Review
6. Records/Reports
Discharge Monitoring Reports (DMRs) and lab data are maintained according to the permit. DMRs
include all permit parameters. Copies of DMRs, operator visitation logs, daily operation sheets,
maintenance reports, lab reports and Chain-of-Custody forms were available for review during the
inspection.
7. Self-Monitoring Program
A review of DMRs from May, 2015 through December, 2017 showed no violations, and a
comparison of submitted DMR data with bench records showed no errors or concerns.
8. Laboratory
The facility analyzes pH,temperature,total residual chlorine, and dissolved oxygen under
certification#5206. Other parameters are analyzed by Water Quality Labs, certification #544.
Copies of all laboratory documentation are kept on site. Procedures for analyzing field parameters
appeared acceptable. Chlorine analysis is performed using a Hach DR2500 spectrophotometer with
the DPD method and using the flow thru cell. A five-point calibration is performed annually.The pH
meter is calibrated daily with two buffers and checked with a third buffer.The DO meter is also
calibrated daily.
However, it was noted that the thermometer's calibration was last checked in April, 2011. Please
note that laboratory certification procedures require that the thermometers used to report
temperature be checked against an NIST-traceable thermometer yearly.A copy of the field analysis
procedure is attached.
During the inspection Mr. Church reported that both composite samplers were malfunctioning. The
influent sampler had stopped working and the operators were compositing by combining 5 or 6
samples taken by hand throughout the day. The sampler had been sent to ISCO and was awaiting
service. The effluent sampler was not receiving signal from the flow meter, so the flow proportional
sampling was down. The sampler was set to sample hourly, and Mr. Church said that he hoped to
have the necessary repairs completed in the coming days.
9. Sludge Handling Disposal
Residuals are land applied under permit WQ0004166.The aerobic digester, belt press and sludge
dryer appeared to be well maintained.
Required Action
Please ensure that the influent and effluent composite samplers are repaired as soon as possible and inform this
office once the repairs have been completed. Future deviations from sampling procedures which may affect the
accuracy of submitted data (for example, inability to take flow-proportional effluent samples)should be reported to this
office as they occur.Additionally, please be sure to complete the thermometer calibration check as outlined in the
attached procedure.

Comment. Non-field parameters are analyzed by Water Quality Labs. Field parameters are measured
in house. Calibration records for pH, DO and chlorine were adequate; however, records
indicated that the thermometer used for temperature reporting was last calibrated in 4/2011.
I reminded Mr. Church that thermometers used for temperature reporting must be checked
against an NIST traceable thermometer at least every 12 months.

Comment Samples are collected at the head of the oxidation ditch. Mr. Church said that his automatic
influent sampler has been broken for the past month or so.They received and
acknowledged an estimate from ISCO to perform repairs on 2/14/2018. In the meantime
they have been compositing manually by taking 5-6 grab samples per day.

Comment: Mr. Church said that the signal wire running from the ultrasonic flow meter to the effluent
sampler recently went bad, so the sampler was not receiving flow signals from the flow
meter. He said they were grabbing a sample every hour and hoped to pet the wire fixed
within the coming days.
