On March 16, 2009, the Division of Water Quality (DWQ) received your application dated March 13,
2009, to fill or otherwise impact 50 linear feet of perennial stream, to install the proposed bypass pump in
the stream at the site. The DWQ has determined that your application was incomplete and/or provided
inaccurate information as discussed below. The DWQ will require additional information in order to
process your application to impact protected wetlands and/or streams on the subject property. The DWQ

information requested below, we will place this project on hold as incomplete until we receive this
additional information. If we do not receive the requested information, your project will be formally
returned as incomplete. Please provide the following information so that we may continue to review your
project.
Additional Information Requested:
1. Please refer to the PCN Checklist located at:
http://h2o.enr.state.nc.us/ncwetlands/documents/PCNCheck]ist vl_4 doc for submittal
requirements.
2. Please provide a photograph of the existing condition of the culvert. Are gabion baskets
necessary or will bioengineering techniques be feasible? DWQ prefers bioengineering to
streambank hardening.
3. Please provide a location map for the project.
4. Please locate your project boundaries on the most recent bound and published Cumberland
County soil survey and the USGS 1:24,000 topographic map for the project.
