Your work is authorized by the above referenced permit provided it is accomplished in strict accordance with the enclosed
Conditions,your application signed and dated 12/20/2017,and the enclosed plans and figures dated 12/20/2017.Any violation
of the attached conditions or deviation from your submitted plans may subject the permittee to a stop work order,a restoration
order,a Class I administrative penalty,and/or appropriate legal action.
This verification will remain valid until the expiration date identified below unless the nationwide authorization is modified,suspended
or revoked. !If, prior to the expiration date identified below, the nationwide permit authorization is reissued and/or modified, this
verification will remain valid until the expiration date identified below, provided it complies with all requirements of the modified
nationwide permit. If the nationwide permit authorization expires or is suspended,revoked,or is modified;such that the activity would
no longer comply with the terms and conditions of the nationwide permit,activities which have commenced(i.e.,are under construction)
or are under contract to commence in reliance upon the nationwide permit,will remain authorized provided the'activity is completed '
within twelve months of the date of the nationwide permit's expiration,modification or revocation,unless•discretionary authority has
been exercised on a case-by-casebasis to modify,suspend or revoke the authorization.
Activities subject to Section 404 (as indicated above)may also require an individual Section 401 Water Quality Certification. You

SPECIAL CONDITIONS
a. A post construction survey of the limits of disturbance to the wetland impact area must be
conducted by a registered Professional Land Surveyor (PLS),within 30 days of completion of work
within jurisdictional waters.This survey must include the as-built footprint of fill material placed for and
in association with the Wetland Impacts 1A, 1D and 1C subject to this Nationwide Permit.verification.
The surveyed as-built fill material footprint must be overlaid on top of the wetland limits as verified by
the Corps of Engineers as well as the limit of"Wetland Impacts" as shown on the enclosed plans entitled
"Wetland Impact—Area 1",revised dated 12/20/2017.
b. Within•30 days of completion of work within jurisdictional waters,the as-built survey, as
described in Special Condition a. above, must be submitted to the Corps of Engineers, along with a
report calculating the amount (acreage) of as-built fill material placed within wetland,limits as verified
by the Corps of Engineers.This report must also disclose any discrepancies between the as-built fill in
wetlands, and limit of"Wetland Impacts" as shown on the enclosed plans entitled"Wetland Impact—
Area 1", revised dated'12/20/2017.
c. Outside of the limit of"Wetland Impacts" and"Stream Impacts" as shown on the enclosed plans
entitled"Wetland Impact—Area 1" and"Stream Impact-Area 2,"revised dated 12/20/2017,the
boundaries of all Potential',Wetland Waters of the US and Potential Non-Wetland Waters of the US as'
shown on the;attached,"Ellis Road Delineation" dated November 2016,prior to any land disturbing
activities. This on-site fencing must be maintained on the property until project construction is completed
and disturbed ground is stabilized. I
Note: Should wetland or stream'impacts exceed the impact limits as defined in this NWP 29 verification
and supporting documentation,then the activity would not be.in compliance with this verification .
