5. Directions to Site: From the junction of NC 150 (west) and E. Maiden Road, take a right
onto E. Maiden Rd. and travel for 7.9 miles until merging onto E. Main Street. Continue
traveling on E. Main Street (later becoming W. Main Street) and turn left onto S. Carolina
Avenue with the intersection of W. Pine Street being in less than a mile. The Mill will be
located directly across from this intersection with the W WTP being past the production
building (once through the security gate).

PART II - DESCRIPTION OF DISCHARGE AND TREATMENT WORKS

Outfall 001-1.1 MGD — industrial. The existing W WT facilities consist of mechanical
screening (lint removal); dual aeration basins with surface aeration; dual secondary
clarifiers; post aeration basin with surface aeration; continuous flow measurement; and
aerated sludge lagoon.

1. Special monitoring or limitations (including toxicity) requests: NA.
PART IV - EVALUATION AND RECOMMENDATIONS
Improvements to the facility's self-monitoring program and overall operation and
maintenance program will be required as a result of the recent inspection (February 22, 2018). The
operations staff continue to use chemical additions for color reduction. This Office recommends the
Permitting Staff to request updated production numbers from the new Owner. It is recommended
that the subject Permit be renewed following review of the updated production numbers and/or
application by the Division's Complex Permitting Staff.
