The Division of Energy, Mineral and Land Resources (DEMLR), received a Stormwater
Management Permit Application for the subject project on December 5, 2017. A preliminary
review of that information has determined that the application is not complete. The following
information is needed to continue the stormwater review:
Repeat comment: The person signing the application, O&M Agreement, and deed
restrictions form must be a member/manager of LGI Homes — NC, LLC as listed with the
NC Secretary of State. Or provide a signed letter from one of the member/managers
listed giving Cedric Combo signature authority. Not in the most recent submittal.
2. Repeat comment: The notary mistakenly signed in the space where the applicant and
property owner are supposed to sign. Please provide a corrected signed original of
SWU-101, 0&M agreement, and the deed restrictions. Not in the most recent submittal
3. Repeat comment: Revise SWU-101 Sect. IV.10: Provide the BUA breakdowns for each DA
in the bottom table. These were left blank. Not in the most recent submittal
4. The plans provided are not printed to scale (Sht. L-3 through L-8, and L-19).
5. The design talcs for sand filter 1 are not consistent with the plans or DA breakdown
talcs. DA and BUA shown as 5 vs. 4.5 and 2.4 vs. 2.16, respectively. Note that for sand
filters, you may choose to use 75% of the WQV per the MDC.

6. Revise the supplement forms:
a. All sand filters: still a few items were left blank (el. of bypass, desc. of bypass,
flow dist., ponding depth from sf top to bypass).
b. Sand filter 1: the design volume minimum and provided are not consistent with
the plans and calcs.
7. Provide pdfs of all revisions, 2 hardcopies of revised plan sheets, and 1 hardcopy of
other documents. Pdfs must be uploaded using the form at:
https://edocs.deg.nc.gov/Forms/SW Project Submittal The pdfs for all revisions were
not submitted for All. Please submit for All and 2.
Please note that this request for additional information is in response to a preliminary review.
The requested information should be received by this Office prior to April 7, 2018 or the
application will be returned as incomplete. The return of a project will necessitate resubmittal
of all required items, including the application fee. Please reference the State assigned project
number SW3171201 on all correspondence.
If you need additional time to submit the information, please submit your request for a time
extension to the Division at the contact below. The request must indicate the date by which you
expect to submit the required information. The Division is allowed 90 days from the receipt of
a completed application to issue the permit.
The construction of any impervious surfaces, other than a construction entrance under an
approved Sedimentation Erosion Control Plan, is a violation of NCGS 143-215.1 and is subject to
enforcement action pursuant to NCGS 143-215.6A.
