Enclosed are copies of the Notice of Violation and assessment of civil penalty letters issued to
NC DOT for the limit violations reported on the Discharge Monitoring Reports from October 2017
through December 2017. The total penalty amount is $5,884 87 ($5,575.00 civil penalty + $309.87
enforcement costs) and NC DOT may issue a single payment of the total penalty amount if the payment
option is chosen All permit limit and/or monitoring violations have now been addressed prior to the
facility's closure in February 2018.
