Please provide building envelopes for all lots with wetlands, streams or buffers (or lots ) on the site plans. ? Please indicate proposed lot layout as an overlay on the site plan. ? Please provide a signed and sealed geotechnical report of subsurface soils at the proposed bottomless culvert location. The report must comply with the Division of Highways - Guidelines for Drainage Studies and Hydraulic Design, prepared by A.L. Hankins, Jr., State Hydraulics Engineer - 1999. ? Your proposed stream crossing appears to be aligned at approximately . Please re-configure your proposed stream crossing to show it aligned approximately perpendicular (between 75° and 105°). Diffuse Flow and Stormwater ? Per the requirements of the Riparian Buffer Rule, you must show that this site meets diffuse flow requirements with a level spreader or other BMP per Chapter 8 of the BMP Manual (see httn://h2o.enr.state.nc.us/suibmp forms.hhn). ? Please submit a Level Spreader Supplement Form with all required items for each proposed level spreader (see httu //n2o.enr.state.nc.us/su/bmp fonns.btm). ? Please show contour lines down slope of the level spreader to show that it has been correctly sited per the BMP Manual. The slope from the proposed level spreader(s) lip ( ) to the top of bank of the stream exceeds the allowable slope for level spreaders (6% for wooded vegetation and 8% for thick ground cover and grass). You must either relocate the level spreader(s) to a location where the filter strip slope(s) is/are appropriate or provide a BMP that removes 85 percent TSS and 30 percent TN and TP. For each proposed BMP, provide a completed BMP Supplement Form with all the required items (see http:hl12o.enr.state.ne.us/su/binp forms.hnn). Please provide calculations to show the outlet velocity from outlet(s) to the wetland(s). A non-erosive velocity of two feet per second or less must be achieved for the peak flow from the 10-year storm. [High Density] Per the requirements of GC , this project is subject to Stormwater Management Plan (SMP) Requirements for Applicants Other Than the North Carolina Department of Transportation (see http://h2o.enr.state.ne.tis/newetlandsn. For each proposed BMP, please provide a completed BMP Supplement Form with all required items (see httn://h2o.enr.state.nc.us/su/bn,p forms.ntm). [Pockets of High Density] Please provide an inventory of imperviousness surfaces in each drainage area. The inventory should include all proposed building footprints, roads, driveways, sidewalks, gravel-surfaced areas, amenity areas, etc. A Stormwater Management Plan (SMP) must be provided for any drainage areas that exceed 24 percent imperviousness. For each BMP, provide a completed BMP Supplement Form with all the required items (see http://h2o.enr.state nc.us/siibmp forms.ntm). Per NCAC 2B 15A: 02H.1000, a "drainage area" is defined as "the entire area contributing surface runoff to a single point." Please provide a BMP Supplement Form with all required items for every BMP on the site (see nrtp:/nt2o.enr.state.nc.uS/su/bmp_fonns.hun). Please use the most current version of the BMP Supplement Form (see: hrto://h2o.em.state.nc.us/su/bmp fonns.htm). Please complete all items in Part I of the BMP Supplement form: Project Information and Design Summary. [For certified local governments only - see 401 unit website] Please provide documentation of the local government's approval of the stormwater management plan (SMP) for this project by providing EITHER: • A valid approval letter and one copy of the approved SMP (including plans and calculations), OR • One set of stormwater plan details and calculations stamped as "Approved" by the local government. Mitigation ? of compensatory mitigation is required for this project. Please provide a compensatory mitigation plan. The plan must conform to the requirements of 15A NCAC 2H .0500 and must be appropriate to the type of impacts proposed. ? Buffer mitigation is required for all road crossings that impact 150 linear feet of riparian buffer or more. Please submit a mitigation plan detailing the amount of buffer impact at crossing and how the impact will be mitigated. ? Please provide documentation from the NCEEP indicating their willingness to accept your request to purchase (type of mitigation) credit to mitigate for the proposed (type of mitigation) impacts listed in your PCN application per section VIII, No. 2. Other ? The application fee is insufficient because over 150 feet of stream and/or over I acre of wetland impacts were requested. Please provide $ This additi al fee must be received before ? a 1'cation can reviewed. Please complete Section(s) on the application. ?C? (L ? Please provide a signed copy of the application. ? Please provide an agent authorization letter for the application signature. ? Please indicate which 404 Permit the USACE has authorized for this project. ? Please provide copies of the application, copies of the site plans or other supporting documentation. ? Please resubmit copies of your information on the January 2009 PCN form or a more recent version. You can download this format the following web address: (see http://www.saw.usace.army.mil/WETLANDS/pcn/) ? Please submit electronic CAD files showing , via email to ian.mcmillangncmail.net and provide a CD (TIFF Group 4 format (black and white, not grayscale or color)) along with your response to the additional information requested. Please provide stream determination documentation on the site (upstream and downstream photos and DWQ stream J identification forms). ? Please provide a qualitative indirect and cumulative impact analysis for the project. Please see DWQ's policy for guidance on our website at: http://h2o.enr.state.nc.us/ncwetlands/documents/InterimintemalPolicyCumulativeImpact.doc ? For areas in nutrient sensitive waters (NSW) or buffered river basins fertilizer specifications for stormwater BMPs should state that that only an initial minimal application of fertilizer to establish the plants shall be used. ? Please indicate whether or not the proposed impacts have already been conducted. ? Please provide proposed septic fields on all (or) lots showing that additional impacts will not be required. Notes: 04-1488v5 Winston Pointe AML Comments March 12, 2009 Per the requirements of GC 3705, this project must comply with Stormwater Management Plan (SMP) Requirements for Applicants Other Than the North Carolina Department of Transportation, available at: http:/'ih2o.eilr.state.tlc.us/ncwetIands/documents; SMPRequiremeiitsforApplicants otherthanNCDOT.pdf If you would like to phase the project with the road as "Phase One," then please include an SMP that removes a minimum of 85 percent TSS and 30 percent TN from the stormwater generated by the road. Then, for the remainder of the project, please provide a conceptual stormwater management plan that includes the following: a. A depiction of the drainage areas on the site. b. An estimation of the percentage imperviousness associated with each of the drainage areas. c. A calculation of the water quality volume associated with each drainage area. d. A scaled depiction of a proposed BMP (state the type of BMP you plan to use) that is adequate to remove a minimum of 85 percent TSS and 30 percent TN from the stormwater generated by the road on the site plan that is correctly sized based on the water quality volume.
