On February 7. 2018, Garcy Ward of the North Carolina Division of Water Resources (NCDWR) Washington
Regional Office conducted an on-site inspection of the replacement of Bridge No. 4 over Aggie Run on VOA Road
in Beaufort County. ln addition to NCDWR staff. David Peasley with RK&K, the on-site inspector was also present
durine the inspection and his time and assistance is �reatly appreciated.
During the inspection, a review of [he NCCi010000 inspection forms revealed they were not being properly
completed and maintained. Deficiencies include:
• lis[ing mul[iple SDO's on a single row and not breaking [hem ou[ separa[ely,
• no SDO's Iisted for the week of January 22, 2018,
• not conductin� a follow up inspection within 24 hrs after a 0.5 in. or greater rain event during the week of
January 29,2018,and
• missing inspection forms.
Similar observa[ions were no[ed and reported ro tlie omsi[e inspector during previous inspections on October 2Q
2017 and January 26. 2018.
The actions described above led to the following violation(s):
I.) �7U1 WQC conditions violation:
The NCDWR issued an approval letter allowin� the impacts for the subject project under General Wa[er Quality
Certification (WQC) No. 408i on May 30, 2017, NCDWR project number 20170407. The approval letter specifies
that the activities must adhere ro the conditions listed in the General Water Quality Certitication, as well as
additional conditions listed in the lettec Condition 8 of WQC No. 4085 states the followino:

The North Carolina Depardnent of Transportation (NCDOT) shall be required to be in full compliance
with the conditions related to construction activities within the most recent version of the'v individual
NPDES (NCS000250) stormwater permit.
Failure to properly to propedy complete and maintain the inspection records represents a violation of
NCS000250 and therefore constitutes a violation of this condition.
2.) NPDES (NCS000250) and NCGO10000 permit violations:
Failure to properly complete and maintain inspection records does not meet the requirements set forth in
NCS000250 Part II, Section CQ )(b)(ii) of NCDOT's individual NPDES permit and the self-inspection and
reporti�g requirements specified in NCGO10000 Section II.B(3).
Accordingly, you are directed to respond to this letter in writing within 30 calendar days of receipt of this notice.
Your response shall include the following:
• List any actions necessary to remediate the violation and prevent it from happening again.
