General Information for City of Salisbury WWTP (Grant Creek
Treatment Train and Town Creek Treatment Train)
January — December 2017
City of Salisbury WWTP consists of two treatment trains. Effluents from the
Grant Creek treatment train and Town Creek treatment trains are
disinfected at each train then combined and dechlorinated at the Yadkin
Effluent treatment train which is located downstream of Grant Creek
treatment train before being discharged at Outfall 001 into the Yadkin
River. Chlorine became primary disinfection in February, 2011 (UV is
secondary). Dechlorination is post disinfection.
Design flow of the Grant Creek Treatment Train is 7.5 MGD, Town Creek
Treatment Train is 5.0 MGD and the City of Salisbury WWTP is permitted
at 12.5 MGD.
The headworks analyses for Grant Creek and Town Creek Treatment
Trains were approved on December 7, 2012. The Industrial Waste Survey
for both treatment trains was conducted in 2014 and approved by PERCS
on October 13, 2014. The Long Term Monitoring plan was approved by
PERCS on June 28, 2017. The Sewer Use Ordinance was approved by
PERCS on January 10, 2013 and adopted by City Council on August 20,
2013. PERCS approved the Enforcement Response Plan on June17,2013.

The City of Salisbury is submitting a revised Long Term Monitoring Plan (LTMP) with the
following revisions:
® Removed Apple Baking Company, Local IU, from LTMP. Permit rescinded
September 30, 2016
a Added Agility Fuel Systems, C1U to LTMP. New permit issued January 4, 2017.
® Arsenic, Cadmium, Lead, and Silver was added to list of parameters analyzed by
contract lab (previously analyzed by in-house lab) due to lower detection limits.
® The following metals detection limits were changed:
Arsenic from 10 pg/L to 2.0 pg/L; Cadmium from 2 pg/L to 0.50 pg/L;
Lead from 5 pg/L to 2.0 pg/L; Silver from 5 Og/L to 1.0 pg/L;
Selenium from 10 pg/L to 5.0 pg/L; and Copper from 5 pg/L to 2.0 pg/L.

Sampling Points and Frequencies:
Salisbury has a unique situation in that we have two Treatment Trains at two separate physical locations
that the SIU's discharge into. The chlorinated effluent from the Town Creek Treatment Train (TE) is
combined with the Grant Creek Treatment Train chlorinated effluent (GE). The combined chlorinated
effluents (YE) join together on the Grant Creek property are dechlorinated, then flow through cascade
aeration, and then discharge into the Yadkin River. Although not permitted as separate WWTPs, Town
Creek and Grant Creek Treatment Trains will be treated as separate WWTPs in the LTMP with data from
both sites used to determine pollutant removal efficiencies and plant inhibition thresholds.
All SIUs and Locals are monitored per IU Permit requirements and all POC's once per year. The City
uses a Yearly Monitoring Plan. The Plan is followed as closely as possible weather permitting. If an SIU
is not monitored in the month indicated, it is monitored as soon as possible.
Uncontrollables are composite and grab samples at point in collection system that serves residential,
commercial (including dentists and other medical), and any unpermitted non-SIUs. Uncontrollable
sampling, though not required, will be monitored annually.
Salisbury will use the actual sludge that is disposed of as our sludge sampling requirement per our sludge
disposal permit (WQ0001956) and EPA 503 regs.

The City of Salisbury's WWTP's annual Priority Pollutant Scan sampled June 13,
2017 experienced an exceedance for the Bis 2 ethylhexyl phthalate (Bis 2)
parameter. After follow-up discussions, inspection, and sampling of AkzoNobel,
the City is requiring AkzoNobel to monitor for the Bis 2 parameter once per
quarter for a year, which began 4t" Quarter of 2017. At the end of the year, 2018,
the city will re-evaluate the data and make suggestions for corrective action(s), if
needed. Note: AkzoNobel has had 'hits" for Bis 2, but compliant with their
pretreatment permit for this parameter.
This industry's IUP was renewed on September 24, 2014. At the request of
PERCS, the IUP was modified on November 13, 2014 to revise the organics
limits and base them on an updated average process flow. The IUP expiration
date is September 1, 2019.
AkzoNobel SPG LLC (AKN) continues to be responsible for the Industrial
Pretreatment Permit compliance for the site. A service agreement between Akzo
Nobel and Henkel delineates the maintenance, operational, and monitoring
responsibilities. AkzoNobel continues to provide the wastewater pretreatment
service for the tenant operations, Indopco, Inc. (Henkel).

Incorrect IDMR dated 8/23/2017. NNC issued 8/23/2017
An Authorization to Construct request was received June 15, 2015 for
installation of two 7,000 gallon reactors and twelve 20,000 gallon raw
materials and waste tanks. The final plans and specifications were reviewed
and approved. Authorization was granted for installation July 14, 2015.
In 2016, One of the (two) 7,000 gallons reactors was brought on-line, and
seven of the (twelve) 20,000 gallon raw materials and waste tanks were
installed and in place--- with four of those in-service. 2017 update: Both
7,000 gallon reactors are now on-line, and seven of the (twelve) 20,000
gallon raw materials and waste tanks are installed in place ---with five of
those in service.
An Authorization to Construct request was received July 26, 2017 for
installation of (Two) 16,000 gallon wastewater storage tanks to replace the
existing two 15,000 gallon storage tanks that were past their service life.
Also some minor modification of piping associated with new storage tanks
to be performed. An authorization to Construct was granted on July 27,
2017. After installing the two 16,000 gallon tanks and modification of
associated piping, an inspection was conducted on September 6, 2017 and
found to be in accordance with the Authorization to Construct. These
storage tanks are now on-line.
This SIU manufactures surfactants and emollients for the personal care
industry and has shown good faith in working with the POTW.The IUP was
renewed effective on October 1, 2017. The IUP expires September 30,
2022.
