Describe the existing conditions on the site and the general land use in the vicinity of the project at the time of this application: General land use in the vicinity is residential and commercial. The project site consists of a single-family residential pond with a surface area of approximately 2.3 acres. The dam of the pond has a damaged outfall structure and a damaged emergency spillway. Shrubs and small trees are currently growing on the dam. Photos documenting the existing condition of the dam are enclosed for review. 3b. List the total estimated acreage of all existing wetlands on the property: There are no wetlands within the limits of disturbance. No wetlands will be impacted by this project. 3c. List the total estimated linear feet of all existing streams (intermittent and perennial) on the property: Currently, there are no streams within the limits of disturbance. No streams will be impacted by this project. Approximately 330 linear feet of stream channel will be un-impounded upon breach of the existing dam. The entire length of the breach and stabilization will be approximately 170 linear feet. 3d. Explain the purpose of the proposed project: The purpose of the proposed work is to breach the existing dam to comply with the Dam Safety Law of 1967. The breach and lake bottom will then be stabilized to prevent erosion. The Division of Land Resources, Land Quality Section issued a "Notice of Inspection" and deficiency letter on July 25, 2007. This letter is enclosed for review. 3e. Describe the overall project in detail, including the type of equipment to be used: The proposed project includes breaching the existing dam and stabilizing the area. Sediment and erosion control measures will be installed within and around the limits of disturbance. The pond will then be drained using a temporary pump and siphon. The old standpipe will be removed and other outfall structures will be capped and grouted. Once the lake has been completely drained, the lake bottom will be seeded and mulched. The dam will then be breached and stabilized. Check dams will temporary be installed along the breach until the breach and pond bottom has been fully stabilized and vegetation begins to grow. Upon completion of the breach and associated construction, all disturbed areas will be vegetated and stabilized. Once the project is stabilized, check dams and all other sediment and erosion control measures will be removed. Typical construction and earth moving equipment will be utilized on site. 4.
