Inspection Type: Site Inspection (DOT) Permit (401 WQC) Does the impact(s) match what was approved in the 401 WQC or non 404 permit? Are the culverts and/or filled areas installed properly? Is the site compliant with additional conditions of the 401 WQC or non 404 permit? Reason for Visit: Routine Yes No NA NE ❑ ❑ ❑ ❑ ❑ ❑ ❑ ❑ ❑ Comment: The proiect was constructed per the plans. An issue reqardinq plastic mesh installation within the Trout Buffer has not been settled with WRC as of this writinq. DEO Div 11 stated it would be too expensive to install core fiber mattinq for all proiects and that plastic mesh is appropriate in that in helps to establish qrass cover quicker and therefore offers more protection from potential in-stream sedimentation. I reauest that he contact WRC to determine if thev are ok with plastic mesh in buffers since their approval was based on DOT not usinq plastic mesh. The trout moratorium was extended bv WRC for one week. Weather prevented proiect completion with the moratorium dates. page: 2
