The North Carolina Department of Transportation (NCDOT) was issued a 404 Nationwide Permit
14 from the U.S. Army Corps of Engineers (USACE) on February 16, 2017 for the abovementioned
project. As construction will continue beyond the expiration date of the permit, a renewal of the
USACE Permit will be required.
NCDOT also was issued Water Quality Certification (WQC) No. 3886 from the North Carolina
Division of Water Resources (NCDWR) on February 17, 2017. Therefore, a renewal of the
NCDWR 401 WQC will also be needed.

There are no changes from the original permit application submitted on December 16, 2016,
therefore, NCDOT requests to renew the permit for this project. Application is hereby made for
renewal of the USACE 404 permit and 401 WQC from NCDWR. We have provided a method of
debiting $240.00 for processing the NCDWR WQC renewal, as noted in the subject line of this
application.
