County. Please fill out the attached Rescission form for Permit No. NCG020417 and send in the signed copy to our
office. The mailing address is on the form. Feel free to also email a copy to Laura Alexander
([e-mail]) so we can begin the rescission process as soon as possible.

I have also copied Deborah Reese to let her know that payments will be ending for this permit. I see in the billing
information for this permit that the invoice you received was #2018PR000863. I’m including this for Deborah’s
reference.
