On October 31, 2017, Glen White and Sue White of the North Carolina Department of
Environmental Quality — Division of Energy, Minerals and Land Resources met with you at
Radford Quarries located at 5605 Bamboo Road in Boone. The purpose was to conduct a
compliance inspection regarding the facilities industrial stormwater permit. Inspection was
requested by Derreck Goddard, PE (Engineer) and yourself following corrective measures put
into place after receipt of an NOV issued to the facility on October 6, 2017 for illicit process
wastewater discharge and compliance evaluation inspection.
Permit:
This facility is covered under General Permit No. NCG 020129 which allows the discharge
of:
• Stormwater point source discharges associated with mining & quarrying of
nonmetallic minerals, mine excavation, processing, and vehicle maintenance;
• Authorized wastewater point source discharges from mining operations;
• Operations of wastewater treatment systems;
• Stormwater and/or wastewater point source discharges from like industrial
activities deemed by the Division of Energy, Minerals, and Land Resources
(DEMLR) to be similar to these operations in the process, or the discharges, or the
exposure of raw materials, intermediate products, by-products, final products, or
waste products.
General Permit NCG020000 became effective on October 1, 2015 and expires September 30,
2020.

Records/Reports:
This facility was required to develop and maintain a Stormwater Pollution Prevention Plan
(SPPP) in accordance with Part III, Items 1 through 9 of the permit. All qualitative and
analytical monitoring records are required to be maintained with the SPPP for a minimum of five
(5) years. The facility has developed an SPPP and has conducted some of the required
qualitative monitoring. Analytical monitoring had not been performed at the time of inspection
but a qualifying rainfall event had not occurred since the issuance of the prior letter.
Facility Sfte Review:
On October 6, 2017, this facility received an NOV following a complaint -driven investigation
that determined that untreated process wastewater was discharging into the South Fork New
River from production processes at the facility. Upon receipt of the violation, Dr. Cecile and his
Engineer, Mr. Goddard initiated corrective action to minimize further damage to the river.
Corrective action taken and site improvements include:
The process wastewater/stormwater basin has been cleaned out, restructured, seeded and
mulch.
• A new upper chamber has been added to the basin that increases the storage volume
significantly. The lower end of the upper chamber includes a forebay going into the
lower basin. Heavy sediments are allowed to settle out in the upper chamber. The upper
chamber serves to help keep the lower basin and the river clean. It is also much easier to
reach for clean-out and maintenance.
• A pre -cast concrete basin box has been installed at the outlet pipe from the gravel wash
equipment and an HDPE pipe has been buried that drains from the box to the basins new
upper chamber.
• Gravel and vegetated berms and swales has been constructed to direct process water from
all areas to the pre-treatment device.
This site appears to-be.in compliance with permit requirements addressed in the NOV �jth the
exception to required monitoring.
Effluent Receiving Waters:
Effluent from this facility drains to the South Fork New River, class "C", "ORC" (Outstanding
Resource) waters of the New River Basin.
Self -Monitoring Program:
The facility is required to provide qualitative and analytical monitoring as per NCG020000, Part
IV, C & D for stormwater, process wastewater and co -mingled discharge. A qualifying rainfall
event has not occurred since the violation was issued. A flooding event had occurred in the days
prior to this inspection but it was not a qualifying event. The banks of the South Fork New River
flooded but the process wastewater basin at the facility did not fill or discharge into the river.
Please provide qualitative and analytical monitoring at the basin outfall at the next qualifying
event. Submit results to the Stormwater Permitting Unit as permit instructs and provide a copy
of the initial result to this office.

Inspection Summary
On October 31, 2017, Glen White and Sue White of the NCDEQ - DEMLR, Winston Salem Regional Office visited the
facility at the request of Dr. DJ Cecile (Owner) and Derreck Goddard (Engineer)to review the facility for compliance following
corrective action taken after receipt of an NOV for illicit discharge
Significant work has been done in correcting site deficiencies The process wastewater/stormwater basin on the production
side of the facility has been cleaned out, a new chamber with a forebay has been added above the existing basin location
Process water drains from the forebay to the lower basin and will discharge at the main basin outfall prior to entering the
river
A pre -cast concrete catch basin box has been installed at the discharge pipe outlet of the gravel wash area and an HDPE
pipe has been installed to route process water directly from washing operations to the new upper chamber of the basin
The site appears to be compliant with requirements of the stormwater permit. Analytical monitoring will still need to be
performed as soon as possible discharge from the outfall begins Monitor quarterly for the first year and semi-annually in
subsequent years as long as results appear to be within parameters.

Comment: Analytical Monitoring is required to be conducted in accordance with the mrmit for facilties that
discharge process wastewater. At the next measurable rainfall event a sample should be
collected at the basin outfall. Submit results to the Stormwater Permitting Unit and col)y to
NCDEQ-DEMLR's Winston Salem Office for verification. Monitor in accordance with
NCG020000, Part IV, Section D in entirety. Include all reauired monitoring narameters. Future

Comment: This facility appears to have corrected the site deficiencies identified in the NOV issued on
October 5, 2017. The permanent basin has been cleaned out, repaired and upgraded to include
an additional upper chamber with a forebay that will catch heavier sediments before Process
water drains into the lower basin. The facility does not appear to have had any wastewater
discha[ge since the inmprovements were made. Sampling can be -gin when a discharge
appears imminent.

The top photo shows discharge pipe during September complaint investigation
and Compliance Inspection. Turbid water is freely pouring onto the ground and
downslope into the New River. The bottom two photos reflect how it looked
during the follow-up inspection in October. Water pours into concrete
containment and is piped into catch basin.
The top photo shows conditions during September compliant investigation and
Compliance Inspection. Berms to direct flow to basin were eroded down
allowing flow down the banks to the New River. The bottom two photos show
the reworked berms directing flow into the catch basins.
These three photos show the catch basins built to handle all flow from the site.
The sides of the basins need to be stabilized but appear to be capturing flow.
