This quarry uses and will use a recycling process water system
in which the waste water is discharged into a recycling tank
to allow sedimentation. The sediment is then introduced into
the final crush and run stone product.. The water is continually
recycled in a closed system which does not. allow any open discharg

The dewatering process involves pumping rain water collected
in the pit itself, after -the same is collected and- settle�d, the clear
water is pumped into a nearby
culvert near the mine site on Bamboo Road.

Water is sprinkled on the road and stockpiles on a regular
basis to prevent dust from rising.
3. Describe in detail the chronological sequence of land disturbing activities and
reference the sequence to the mine map(s). Attach additional sheets as needed.
This mining area was originally commenced in approximately 1974 by
owners prior to Radford Quarries and the Cecile family. Since 1981
when the initial permit was issued to Radford Quarries, the company
has mined within the now existing perimiters of the mine si,.te.
Mining activities now are within the established perimeters for
the mi6e and will be for some Lime Lo come. There remains a
substantial volume of stone within the perimeter and the land
disturbing activities will be wiLhin that perimeter. It is
anticipated that some benching may be necessary on the east highwall
perimeter'in 1993 or 1994.

b. Describe other meLhods to be taken during mining Lo prevent physical hazard
W any neighboriLng dwelling house, public road, public, commercial or
iridusLrial building from any mine oxcavaUon. Locate all such structures on the
mine map if they are within 300 feet of any proposed excavation.
As noted herein the company uses strict drilling safety measures
to prevent fly rock from escaping the pit area. No commercial
or residential dwellings are within 1000 feet of the mine.
Travelers on Bamboo Road are stopped and flagged approximately
1/4 mile from the mine siLe during blasting activities.
c. Describe whaL kind of barricade will be used Lo prevent inadvertent public
entry along any high wall area and when it will be implemented.
There exists already a barricade along the high wall which
adjoins the Watauga County landfill and hat been for some
six years. At present, there is a berm consisting of a eight
to twelve foot pile of rip rap, which is interspersed with
vegetation. No problems with inadverLant public entry has
been experienced in the last six years. However applicant
intends to erect a barbed wire fence to provide additional
protection to the public.

S. Describe specific erosion and sediment contral measures Lo be taken during
mining to prevent offsite sedimentation (include specific plans for erosion
cuntrol for wine excavaLiLon(s), waste piles, access/mine roads and process areas),
and give a detailed sequence of insUillation. Locate and label all erosion and -
sediment control measures (in Lhe mine map. Lngincering designs and calculations
slidll be required when needed to jusLify the adequacy of any proposed measures. -
1. Mine area - At the present Lime applicant has had no problem
with sedimentation or erosion problems aL the mine site.
However rip rap and surge rock is maintained around the
excavation area to prevent any off site sedimentation. A
number of white pines have been planted along the Bamboo Road
to both screen and prevent runoff from the mine site itself.
2. Process area -Applicant's process area, owned by Raymond ----
Cecile which property is across Bamboo Road from the mine
borders on the New River. Surge rock and rip rap have been
placed on the process area to prevent any run off or erosion
toward the New River. Applicant submits that this procedure
has been more effective than placing silt screens on the
property.
G a. Describe meLhods to prevent landslide or slope instability adjacent to
adjoining properties during mining.
Applicant uses the bench or terrace method for preventing
landslides adjacent to the mine site.
b. In excavation(s) of unconsolidated (non -rock) materials, specify the angle
of all slopes including specifications for benching and sloping.
Applicant uses a three to one ratio on all slopes. Very little
unconsolidated materials border this particular mine.
c. in hardrock evacations, specify proposed bench widths and heights in feet.

8. Describe specific plans (including a schedule of implementation) for screening
the operation from public view such as maintaining or planting trees, bushes
or other vegetation, building berms or other measures. Show the location of
all visual screening on the mine map and provide cross-sections through all
proposed berms.
Applicant has planted white pines around the rim of the mine and
these trees are now substantial in height. The upper wall, as
noLed has a subsLantial berm in place interspersed wiLh vegatation.
A natural screen exisLs along Bamboo Road including Lrees which
totally blocks the mines siLe from the motoring public, except for
the mine road which enLers Lhe mine siLe.
9. Will explosives be used? Yes X No . T1- yes, specify the types
of explosive(s) and describe what precaution(s) will be used to prevent
physical hazard to persons or neighboring property from flying rocks or
excessive air blasts or ground vibrations. Locate the nearest offsite
structure(s) to the proposed excavation(s) on the mine map.

2. Fly rocks, excessive air blasts and ground vibrations are
prevented by the drill pattern and the amount of explosive
placed in the drill hole. ApplicanL loads each shot at a
depth more than fourLeen feet from the top of the hole. This
procedure almost toLally prevents fly rock from leaving the
interior of the mine.

D. �RECLAMATION PLAN
1. Describe your int,endcd plan for Lhe reclamation and subsequent use of all
affvcLed lands and indicaLe Uio general meLhods to be used in reclaiming this
land. This informaLion must be illusLrated on a reclamation map.
1. Overburden around the quarry will be graded to a stable
slope and revegetaLed or rip rapped.
2. The quarry will be allowed to fill with water if a suitable
reservoir can be created.
3.. The plant area on Bamboo Road will be graded level and
stablized.
A permanent berm barrier will be installed along the upper
perimeter of the quarry highwall, including vegetation.
5. The stockpile area adjacent to New River will be graded
level and either stabilized for commercial use or
revegetated. The sideslopes of the stockpile area fill
will be graded to a stable grade and either revegetated
or rip rapped.
6. All areas will be graded to prevent the collection of
standing foul water.
7. No contaminants will be dumped at the mine site. Scrap
meLal, trash or other such waste will be removed from the
site and properly disposed.
2. a. Is an excavated or impounded body of water to be left a:; a part of the
reclamation? Yes X - No . If yes, illustrate the location of the body(s)
of water on the reclamation map and provide a scaled cross-section(s)

b. Describe provisions for prevention of noxious, odious or foul water
uollecting or remaining in mined areas. Provide details on any permanent
water outlets.
All areas of the mine site and processing plant site will be
graded and back-filled to reduce or eliminate any areas which
would collect standing water. A water outlet may be necessary
for the impounded water left in the mining permit but is not
planned at this time.

RMCLAMATION PLAN
3. Describe provisions for safety to persons and to adjoining property in all
completed excavations in rock including what kind of permanent barricade
will be left. The location of the barricade must be shown on the reclamation
map.
A permanant rip rap berm will be placed on the upper rim to
Prevent persons from coming to the high wall edge. Vegetation
exists on all other borders of the mine site including the side
along Bamboo Road. These areas will be enhanced by additional
vegetation and rip rap berms as needed to reduce entrance to the
mining area.
4. a. Indicate the method(s) of reclamation of overburden, refuse, spoil banks or
other such on-site mine waste areas,,including specifications for benching
and sloping.
All overburden and other on site waste areas will be graded
level or to a stable slope of no more than a 3 to 1 ratio and
then sLablized WiLh ground cover such as grass or shrubbery to
prevent erosion. Those areas which are benched will be graded
and sLabilized so as to prevent, slides and erosion.
b. Are any processing waste, overburden or other such mine wastes to be disposed
of off-site? Yes No X If yes, describe in detail what these
wastes are and how they will be disposed. Attach a separate site map(s) showing
the location(s) of the disposal area(s). Include all specifications for erosion
and sediment control.

All processing areas will be graded flat. and any overburden
or wasLe product will be used Lo back -fill in Lhe pit area.
The area near the New River will be graded, stabilized and
sown pursuant to the schedule in paragraph nine hereafter.
All roadways which may be eliminated will be eliminated;
those which cannot will be graded graveled and ditched.

All scrap metal on site will be recycled to area scrap iron
dealers. No contaminants are used on site except maintenance oils
used in repair of mining vehicles. These oils are and will be
disposed properly.
8. Indicate the method of restoration or establishment of any permanent drainage
channels to a condition minimizing erosion, siltation Lind other pollution.
Give design detaiLs including typical cross-sections of any permanent channels
to be constructed cis part of the reclamation plan. Show the location(s) of ail
permanent channels on the reclamation map.
It is not anLicipated, once the grading is completed and the area
stabilized and planted that any permananL drainage channels will be
needed. However as the mine operation conLinues such channels
may be necessary for proper erosion control.

RECIJMTION PLAN
I
10. Provide a schedule of reclamation that indicates the sequence of
reclamation and approximate time frame. If reclamation is to be accomplished
concurrently with mining, then show on the site map segments that are to be
mined and reclaimed during each year of the permit. Add drawings showing typical
sections or cross-sections and layout of proposed reclamation.
It is anticipated that the mining process will continue for a
number of years into the future. Reclamation will occur -
concurrently as areas of the mine are exhausted.
U. NOTIFICATION OF ADOOIWING LANDOWERS
1. The "NOTICE" form or a facsimile thereof attached to this application must be
sent certified or registered mail to all adjoining (contiguous) landowners'
indicated on mine map unless another means'of notice is approved in advance
by the Director, Division of Land Resources'.
The completed "Affidavit of Notification" attached to this application must be
submitted before the application is considered complete.

APPLICATION FOR A MINING PERMIT
G. LAND ENTRY AGREEMENT
We hereby grant to the Department or its appointed representatives the right of
entry and travel upon our lands or operation during regular business hours for the
purpose of making necessary field inspections or investigations as may be reasonably
required in the administration of this Act.
We further grant to the Department or its appointed representatives the right
to make whatever entries on the land as may be reasonably necessary and to take
whatever actions as may be reasonably necessary in order to carry out reclamation
which the operator has failed to complete in the event a bond forfeiture is
ordered pursuant to G.S. 74-59.
