RE: Rexham Packaging Systems, Inc./Plastek Industries, Inc.
Dear Sir/Madam:
I am enclosing and submitting on behalf of Plastek Industries, Inc. a Stormwater Permit
Ownership Change Form along with a copy of the deed reflecting the ownership change.
Please note that ownership changed on April 22, 2010. Apparently, no ownership change form
was submitted at that time although the annual fee has been paid each year and is current. Since
so much time has lapsed since the ownership change, and since the prior owner has been
acquired by another company, it will be difficult if not impossible to obtain the signature of a
representative of the permit holder prior to such change. I respectfully request that you accept
the application as submitted.

designation Grantor and Grantee as used herein shall include said parties, their heirs, successors,

There is included in this conveyance (1) the Grantor's right
privileges and benefits and the rights, privileges and
benefits of and for the Property contained in the agreements
