It sounds like a rescission form should have been filled out. Did you move to another location in NC? If not, then please
fill out the attached form and the regional office will schedule an inspection at the location.

Minuteman Powerboss no longer is at that address in Aberdeen as of August 2016. I spoke to a woman over the phone
last year about this and she stated she understood. I apologize I don’t remember her name to help you find any leads.
We were renting at that location I believe she was going to contact the actual owner of the building about this. Let me
know if this helps or if you need me to do anything.
