The North Carolina Division of Water Resources conducted an inspection of the Mountain View
Pediatrics LLC Facility on 2/8/2018. This inspection was conducted to verify that the facility is
operating in compliance with the conditions and limitations specified in NPDES WW Permit No.
NCG550977. The findings and comments noted during this inspection are provided in the
enclosed copy of the inspection report entitled "Compliance Inspection Report".
There were no significant issues or findings noted during the inspection and therefore, a
response to this inspection report is not required.

On February 7, 2018, Tim Heim of the Asheville Regional Office conducted a compliance evaluation
inspection of the subject facility. No foam or other debris were observed in the receiving stream. The
system appeared to be maintained and functioning properly at the time of the inspection.
The subsurface drainfield appeared to be well maintained and did not show evidence of ponding.
The effluent pipe was not located, this may have been due to high water levels in the receiving stream
at the time of the inspection. The permittee should locate, and maintain access to, the effluent pipe.
It is recommended that the septic tank be pumped every 3-5 years. Records of the septic tank
pumping events should be kept for future compliance inspections.
