Dear Mr. Davis,
Please find attached one copy of a proposed waste pit reclamation plan as submitted by the prime
contractor, Wright Brothers Construction Company, Inc. on the above referenced project. The
Contractor proposes to haul waste rock material from NCDOT project 6.759002T, Etc. (R-529
BA & BB) to Radford Quarries. This waste rock will be crushed by Radford Quarries and sold to
the public.
As discussed in our phone conversation on the above referenced date, this office will not approve
this reclamation plan until we hear from you that your office has modified the active permit at
Radford. Thanks for all of your help in this matter.

All mine operators in North Carolina must submit an Annual Reclamation

calendar year 2000 must be submitted by February 1, 2001.

Two copies of the f6mi are enclosed. Please complete both copies of ef
and return them with two copies of an updated mine map (except as provided in the
following paragraph) for each permit. The map may be an aerial photograph,
topographic map or plan map that accurately gives all information requested on the
Annual Reclamation Report form. The map should be prepared according to the
following specifications:

If your operat ion was idle in 2000, or no new acreage was affected by rn inin g,
and no areas reclaimed in 2000, you do not have to send an updated mine map. Also,
if you have complet6d a permit action since January 1, 2000 (which involved updating
m e map), you do not have to send an updated mine map. However, th e blue

used not only for evaluating the reclamation progress at each mine, but for compiling

map not previously approved by this office will require a formal mining permit modification. You must
submit the modification request separately to receive approval before conducting the proposed activity.
