REQUEST FOR MORE INFORMATION
Dear Mr. Bartlett:
On December 30, 2008, the Division of Water Quality (DWQ) received your revised
application dated December 29, 2008 to impact 3.92 acres of waters, 160,223.8 square
feet (ft) of Zone 1 protected riparian buffers and 98,749.8 square feet (ft) of Zone 2
protected riparian buffers for the purpose of stream restoration. The DWQ has
determined that your application was incomplete and/or provided inaccurate information
as discussed below. The DWQ will require additional information in order to process
your application to impact protected wetlands and/or streams on the subject property. If
you want to carry out this proposed impact, you will need to submit the requested
information within 30 days of receipt of this letter.
Additional Information Requested:
1. Appropriate request for Certification
On February 2, 2009 your consultant took park in a scoping meeting at the
Washington Regional Office. Present at this meeting were representatives from
the US Army Corps of Engineers (COE), Division of Land Quality and the DWQ.
In that meeting the COE stated that they might request the project obtain an

Individual Permit because the project parameters fell outside of the Nation Wide
(NW) permitting process. Please provide information indicating which permitting
process the COE will require to review your request to impact Waters of the US.
This letter only addresses the application review and does not authorize any impacts to
wetlands, waters or protected buffers. Please be aware that any impacts requested within
your application are not authorized (at this time) by the DWQ. Please call Kyle Barnes

up a meeting to discuss this matter.
