On December 21, 2017, the Division of Water Resources (Division) received your application
dated December 20,.2017 requesting a 401 Water Quality Certification from the Division for
your project.. The Division has determinedthat your application is incomplete and cannot be
processed. The application is on-hold;until the followiing,information is received:
1. Provide a legible overall site plan. The impact amounts and footprint of impacts
must be depicted numerically, spatially clear, legible, and clearly identified on the
map. These impacts must correspond to the to,the impacts requested in the
application. [15A NCAC 02H .05,02(g)]
2. The Corps of Engineers must also verify the Nationwide Permit that is to be used to
authorize this project. [15A NCAC 02H .0502(a).(7)]

Pursuant to Title 15A NCAC 02H .0502(c), the applicant shall furnish all of the above requested
information for'the proper consideration of the application. If all of the requested information_____
is not received in writing Within 30 calendar days of receipt of this letter, the Division will be -
unable to approve the application and; it will be returned. The return of this project will
necessitate reapplication to the Division fo'r approval, including a complete application package
and the appropriate fee.
