Dear Mr. Desai:
The City of Greensboro (City) proposes to construct a new ash clarifier and pumping
facility to replace the facilities at the T.Z. Osborne Water Reclamation Facility (NPDES
number NC0047384). The existing ash clarifier and thickened ash slurry pump station
are reaching the end of their useful life, and the proposed work will improve the
reliability of the ash thickening facilities.
The proposed ash clarifier will treat discharges from the existing biosolids incinerators/
quencher/scrubbers and the ash dewatering equipment located in the solids handling
building. Thickened underflow from the clarifier will be pumped to the existing rotary
press type ash dewatering equipment, or to a new belt filter press being designed (by
others).
On behalf of the City, we request an Authorization to Construct for the work described in
the enclosed application for Authorization to Construct approval.
The construction timeline for the implementation of the project is summarized below:
Spring 2018 - anticipated bidding
Summer 2018 - anticipated notice to proceed for construction contractor
Summer 2019 - anticipated completion of construction

SECTION 1: INSTRUCTIONS AND INFORMATION
A. The Division of Water Resources will accept this application package for review only if all of the items are provided and the
application is complete. Failure to submit all of the required items will result in the application package being returned as incomplete
per 15A NCAC 02T .0105(b).
B. Plans and specifications must be prepared in accordance with 15 NCAC 02H. 0100, 15A NCAC 02T, North Carolina General Statute
133-3, North Carolina General Statute 143-215.1, and Division of Water Resources Minimum Design Criteria for NPDES
Wastewater Treatment Facilities.
C. The plans and specifications submitted must represent a completed final design that is ready to advertise for bid.
D. Any content changes made to this Form ATC -12-14 shall result in the application package being returned.
E. The Applicant shall submit ONE ORIGINAL and ONE DIGITAL COPY (CD) of the application, all supporting documentation and
attachments. All information must be submitted bound or in a 3 -ring binder, with a Section tab for each Section, except the
Engineering Plans.
F. Check the boxes below to indicate that the information is provided and the requirements are met.
G. If attachments are necessary for clarity or due to space limitations, such attachments are considered part of the application
package and must be numbered to correspond to the item referenced.
H. For any project that requires review under the State Environmental Policy Act (SEPA), an Authorization to Construct cannot be
issued prior to the completion of a State Clearinghouse advertisement period for a FONSI, EIS, etc. unless the project qualifies for
a Determination of Minor Construction Activity.

D. PROJECT DESCRIPTION
Provide a brief description of the project: The City of Greensboro owns and operates the T. Z. Osborne (TZO) Water Reclamation
Facility (WRF). The City disposes of all residual solids produced at the TZO WRF by thickening, dewatering, and incineration. Two
fluidized bed incinerators are used to incinerate the sludge. Water is used to scrub ash particles from the off -gas prior to
discharge into the atmosphere. The ash slurry produced by the scrubbing process is recovered using an ash clarifier. The existing
ash clarifier and thickened ash slurry pump station are reaching the end of their useful life. In order to improve the reliability of
the ash thickening facilities, the City proposes to construct a new clarifier and pumping facilities to replace the old facilities.
The proposed ash clarifier will treat discharges from the existing FBI No. 1 and No. 2 quencher/scrubbers and the ash
dewatering equipment located in the solids handling building. Thickened underflow from the clarifier will be pumped to the
existing rotary press type ash dewatering equipment, or to a new belt filter press currently being designed (by others).

A. Cover Letter

that indicates whether the project is a new facility, facility modification, treatment process modification, or facility expansion;
the construction timeline; and a list of all items and attachments included in the application package.

❑ Pollutant loading for each treatment unit demonstrating how the design effluent concentrations in Section 4.6.2 of this form
were determined.
❑ Hydraulic loading for each treatment unit.
❑ Sizing criteria for each treatment unit and associated equipment (blowers, mixers, pumps, etc.)
❑ Total dynamic head (TDH) calculations and system curve analysis for each pump specified that is included in Section 4.C.6.
❑ Buoyancy calculations for all below grade structures.
❑ Supporting documentation that the specified auxiliary power source is capable of powering all essential treatment units.

Section 4
Residuals Management Plan
At the T.Z. Osborne WRF, primary and waste activated sludge is incinerated in a 65 dry ton per day
fluidized bed incineration (FBI) system located in the solids handling building. Dewatered sludge
from the centrifuges is conveyed to sludge hoppers through a series of troughed belt conveyors.
Sludge piston pumps feed sludge into the FBI system. The FBI uses No. 2 fuel oil as the primary fuel
in the reactor vessel, with natural gas as a backup. A Hydro -Sonic Systems tandem nozzle scrubber
is attached to the outlet of the new FBI to treat exhaust gases. Ash slurry is dewatered and land-
filled at the City landfill.
Based on historical plant data, typical blended solids characteristics are:
Average dry solids from dewatering: 27.9%
Average volatile solids:
73.2%
Typical solids heating value of volatile solids: 10,000 Btu/lb
Typical solids heating value on a dry solids basis: 7,320 Btu/dry lb.
The City has an air quality permit for operation of the FBI (ES -20) which is AP No. 04489R20.

The undersigned, having visited the Site of the proposed Work, read and understood the
Bidding Documents, , and all reports and drawings of existing subsurface, if any, and
physical conditions at the Site, and all reports and drawings of a Hazardous
Environmental Conditions (if any) at the Site, as indicated in the Supplementary
Conditions, and understanding Laws and Regulations applicable to the Work, agrees to
provide all labor, equipment, materials, and other work necessary for and to construct the
Work in the manner required by the proposed Contract Documents; and pursuant to the
Bidding Requirements, submitted herewith a bid security in the form of a certified check
for the sum of dollars, or bid bond in the amount of
percent of the amount bid payable to the City of Greensboro, North Carolina, under
conditions indicated in the Bidding Documents. The Bid is valid and subject to the
Owner's acceptance for the period of time indicated in the Instructions to Bidders for this
Project.
2. The undersigned proposes to enter into a Contract in accordance with the Bidding
Documents for the prices shown on the pricing sheets that are part of this Proposal.
3. It is understood that all quantities of Unit Price Work are approximate and subject to

4. It is understood that the City reserves the right to reject any or all bids and to award the
contract in the best interest of the Project.
5. The undersigned certifies and agrees that the following information is correct in reference
to the City of Greensboro's M/WBE Plan for Construction, Professional Services, Goods
and Other Services, established October 15, 2013, by the Council:
a. In preparing the Bid, the Bidder has considered all proposals submitted from
qualified, potential Subcontractors and suppliers, and has not engaged in
"discrimination" as defined in the City's M/WBE Program Plan, Section V. A. 1.; to
wit: discrimination in the solicitation, selection or commercial treatment of any
Subcontractor, vendor, supplier or commercial customer on the basis of race,
color, religion, ancestry or national origin, sex, age, marital status, sexual
orientation or on the basis of disability or other unlawful forms of discrimination.
Without limiting the foregoing, "discrimination" also includes retaliating against any
person or other entity for reporting any incident of "discrimination". Without limiting
any other provision of the solicitation for responses on this project, it is understood
and agreed that, if this certification is false, such false certification will constitute
grounds for the City to reject the Bid submitted by the Bidder on this project, and
terminate any contract awarded based on the response. As part of its response,
the Bidder shall provide to the City a list of all instances within the immediate past
4 years where there has been a final adjudicated determination in a legal or
administrative proceeding in the State of North Carolina that the Bidder
discriminated against its Subcontractors, vendors, suppliers or commercial
customers, and a description of the status or resolution of that complaint, including
any remedial action taken. As a condition of submitting a Bid to the City, the
Bidder agrees to comply with the City's Commercial Nondiscrimination Policy as
described under its M/WBE Program Plan, Section V. A. 1.
6. In default of the performance of any of these conditions on our part to be performed, the
bid security attached hereto shall at the option of the City Council become the property of
the City of Greensboro, N. C., as ascertained and liquidated damages for such defaults;
otherwise the said bid security is to be returned to the undersigned or destroyed.
7. Time of Completion - Bidder agrees that the Work will be substantially complete and
completed and ready for final payment in accordance with Paragraph 14.07 of the
General Conditions on or before the dates or within the number of calendar days
