Sullins Dee Freeman Governor Director Secretary February 9, 2009 EXPRESS REVIEW ACCEPTANCE LETTER NECHS Attn: Mr. Dick Pontello 1507 Seabrook Avenue Cary, NC 27511 Project Name: Smith Addition (PIN: 0744663038) Dear Mr. Pontello: Wake County On February 9, 2009, the 401 Oversight/Express Permitting Unit of the Division of Water Quality received a request from Mr. Leif Harris, regarding a project known as "Smith Addition" for acceptance into the Express Review Program. This letter advises you that your project will be accepted into the Express Review Program once the following items are received: 1) The fee of $200.00 made payable to the North Carolina Division of Water Quality (for one (1) stream origin determination); 2) Two (2) complete and collated copies of the most recent versions of the NRCS Soil Survey map and the 1:24,000 USGS Topographic map for the site, both with the property boundaries located; and 3) A vicinity map clearly marking the location of the site. Thank you for your attention to this matter. The clock for this project will not start until receipt of the completed application package and required fee are received. If the impacts are greater than indicated on the Initial Interest Form and/or the services are different, you may be required to remit an additional fee.
