On November 28, 2017, the Division of Water Resources — Water Quality Programs (Division) received your
application dated Novmeber 20, 2017, requesting a 401 Water Quality Certification from the Division for
your project. The Division has determined that your application is incomplete and cannot be processed.
The application is on -hold until all of the following information is received:
1. The owner of the project is listed as the Uwharrie Green Schools Inc., however the agent
authorization form has been signed by a representative of Uwharrie Charter Academy. Please
provide an agent authorization form that is signed by an appropriate representative of the owner of
the project.
Based on a site visit conducted on January 3, 2018 with Pilot Environmental, please modify the
jurisdictional features map to indicate that stream ISC is a perennial channel below the confluence
with stream ISCA.
3. Per Condition 1.5 of General Certification #4139, this project is required to provide either:
a stormwater management plan, or calculations to document that this project will not
cause degradation to downstream water quality. Please see the attached GC44139 for
specific details of this requirement. [15A NCAC 02H .0506(b)(5) and (c)(5)]
4. Based on the design documents provided, it appears that the culvert outlet at Impact #1 is
aligned so that the flow is directed at a streambank within a meander bend of the channel.
In order to protect the stream channel from erosion and sedimentation, please realign the
culvert, or propose a channel realignment below the culvert to ensure there are no long
term water quality issues as a result of the proposed impact.

Pursuant to Title 15A NCAC 02H .0502(e), the applicant shall furnish all of the above requested information
for the proper consideration of the application. If all of the requested information is not received in writing
within 30 calendar days of receipt of this letter, the Division will be unable to approve the application and it
will be returned. The return of this project will necessitate reapplication to the Division for approval,
including a complete application package and the appropriate fee.
