On December 13, 2017, Paul DiMatteo and Jim Gonsiewski of the Division of Water Resources
("Division"), Winston-Salem Regional Office conducted a Compliance Evaluation Inspection of the above
referenced wastewater treatment system. The presence and cooperation of Ms. Melinda Ward and Mr. Chris
Powell was greatly appreciated. This type of inspection consists of two basic parts: an on-site inspection of the
facility and a review of facility files and self-monitoring data. Observations from each area of the inspection
are summarized below.
Site Review
1. Permit
The current permit became effective on March 1, 2015 and expires on April 30, 2018.
A permit renewal application was received by the Division on November 1, 2017. A copy of the
current permit was available for review. Permitted components include bar screens, two aeration
basins, four clarifiers, gas chlorine disinfection and dechlorination.
2. Facility Site Review
The facility was as described in the permit, and is operated 24 hours per day, 7 days per week. At
the time of inspection, the two larger clarifiers and one aeration basin were offline due to
prolonged low flows. The operators said that an emergency generator at the nearby water
treatment plant is capable of powering both facilities.
3. Operation and Maintenance
Melinda Ward is the current Operator in Responsible Charge, and Chris Powell, Glenn Pulliam and
Ronald Wright are backup ORCs. All operators are properly designated and hold the correct
credentials. The facility was very clean at the time of the inspection and appeared to be well

4. Flow Measurement
Influent flow is measured using an ultrasonic device set above a stilling well located next to a
Parshall flume. The flume is completely covered with a grate except for a section above the stilling
well. The stilling well appeared to be located next to the throat of the flume. The reading from the
stilling well should be checked against the standard measurement location to ensure that reported
flow is within 10% of the true value. The ultrasonic device's calibration was last checked on
November 22, 2017 by Instrumentation Services, Inc., and is usually checked quarterly.
5. Effluent/Receiving Waters
The effluent is discharged to the Dan River, Class WS -IV waters in the Roanoke River Basin. The
outfall was not observed during the inspection due to accessibility issues.
Documentation Review
6. Records/Reports
Discharge Monitoring Reports (DMRs) and lab data are maintained according to the permit. DMRs
include all permit parameters. Copies of DMRs, operator visitation logs, daily operation sheets,
maintenance reports, lab reports and Chain -of -Custody forms were available for review during the
inspection.
7. Self -Monitoring Program
A review of DMRs from August, 2016 through October, 2017 showed no violations, and a
comparison of submitted DMR data with bench records showed no errors or concerns.
8. Laboratory
The facility analyzes TSS, fecal coliform, pH, temperature, and DO under certification #168. Other
parameters are analyzed by Pace Analytical. Copies of all laboratory documentation are kept on
site. Procedures for analyzing field parameters appeared acceptable.
Composite samplers are programmed to be time -proportional (one aliquot every 15 minutes).
Please note that item (4) under the definition of "Composite Sample," Permit Part II Section A
states that use of constant time,tconstant volume composite sampling requires prior approval by
the Director and may only be used in certain circumstances. Otherwise, sampling should be flow -
proportional.
9. Sludge Handling Disposal
Sludge is currently held in an aerated basin until it is hauled off site for further processing. The
operators said that the facility was planning on overhauling its sludge handling processes.

Site Review

Comment: There are 5 bar screens on site Two are located near the influent, before grit removal Two
were placed at the end of the aeration basins because they were having
issues with smaller
debris getting through the Initial screens and accumulating into larger masses In the aeration
basin The use of these bar screens is being phased out. A newer, fine bar screen was
installed after the initial bar screens, which the operators said has greatly
helped the
accumulation issue.
