Eric and Rob,
A permit application for this project was sent to you via email on April 8, 2016, My
records do not indicate that the USACE ever issued a nationwide permit project. The
NCDENR-DWR issued a 401 WQC and buffer authorization on May 9, 2016. Approval
for construction of this project is authorized by the USACE until March 18, 2018.
Work will not even begin at this culvert until late Spring, 2018 and work in the stream
will not be completed until late Fall, 2018. Accordingly, the Department requests that
you issue new permits for this project. There have been no changes to the construction
plans or permit drawings. I have been on site recently and there appear to be no
changes to any jurisdictional features identified at the project.
Please advise if you need additional information.

NCDOT Division 5 proposes to replace Bridge No. 310093 over Third Fork Creek on SR
1945 (South Alston Avenue) in Durham County. Note that this is part of the State
Funded Design Build Year II Set A program. The existing bridge is being replaced with an
aluminum box culvert. The following information is attached as part of this permit
application:
-Signed and scanned copy of PCN form
-In-lieu fee mitigation acceptance letter
-Combined Stream/Wetland and Riparian Buffer Permit Drawing Package
