A review of the October 2017 Discharge Monitoring Report (DMR) for the subject facility revealed the violation
indicated below:

The Division of Water Resources requests further information regarding the noted violation detailing why an additional sample
was not immediately collected after the laboratory accident occurred. Please respond in writing within fifteen (15) business
days after receipt of this Notice. A review of your response will be considered along with any information provided on the
submitted Monitoring Report.
Remedial actions, if not already implemented, should be taken to correct any noted problems. The Division of Water Resources
may pursue enforcement actions for this and any additional violations. If you have any questions concerning this matter, please
