Enclosed is a 401 Water Quality Certification Application for your review. It is submitted on behalf
of the City of Kinston for the repair of a stream bank located at 1708 W. Washington Avenue in
Kinston, NC (Lenoir County). The bank was damaged during Hurricane Matthew and has eroded to
the point where it is only a few feet from the existing house on the lot and the house is no longer
habitable. The project will stabilize 102 linear feet of the bank by installing a geotextile fabric and
rip rap.
Enclosed are the PCN form, site maps, and the site plan. Copies of the application and the DWR
application fee have been submitted to your Raleigh Office. Please let me know if you have any
questions or need additional information in order to issue the 401 WQC. Thank you for your

3a. Describe the existing conditions on the site and the general land use in the vicinity of the project at the time of this
application:
Project area is currently a residential lot with a home on it. A perennial stream is located along the western boundary of
the lot. This stream is approximately 5' wide and 10' deep and contains various trash and debris. Hurricane Matthew
eroded its banks and the eastern bank is only a few feet from the existing home (see attached pictures). This home is
currently uninhabitable because of the erosion. Adjacent land use is residential or industrial.

The purpose of the project is to restore the eastern bank of the stream to a 1.5 to 1 slope so that it is stable and the home on
the property can be habitable. This will reduce erosion entering the channel as well.
3e. Describe the overall project in detail, including the type of equipment to be used:
This project is FEMA-funded to the City of Kinston to repair erosion caused during Hurricane Matthew on private property. A
temporary diversion will be placed in the channel to divert flow while the toe is being installed. The stream bank will be
regraded and geo-textile fabric that will extend from the stream channel to the top of bank will be installed. Approximately 24"
of Class 1 rip rap will be installed on top of the fabric. A liner will be placed on top of the rip rap. The top of bank will be
seeded with centipede grass. Track hoe, loader, dump truck, bob cat, and/or other standard construction equipment will be
used. Equipment will work from uplands.
