We are also enclosing a check for $100 for the required permit fee. Altec Industries, Inc. owns and operates the facility that serves as a manufacturing assembly and service operation in Mount Airy, North Carolina (Altec Mt. Airy). The facility is located at 200 Altec Way, Mount Airy, North Carolina, in Surry County. The facility services aerial lift trucks, and assembles chassis and trailers for use by utility companies and the tree care industry. The facility is located approximately 1,300 feet southwest of Reeves Mill Road, and is approximately 4.5 miles southeast of downtown Mt Airy, North Carolina. Altec Mt. Airy's property in comprised of approximately 90 acres and includes three single -story metal buildings originally built around 1998. Bordering the facility's western property boundary is the Mt. Airy-Surry County Airport and aircraft hangar and bordering the northeastern boundary is Pike Electric, LLC property. Altec Mt. Airy has paved driveways and parking areas surrounding the Altec buildings that include a storm drain system that drains to the south and southwestern portions of the site. There are three terraced gravel -covered staging areas on the southern portion of the property. The facility also receives stormwater runoff from the adjacent, northeastern Pike Electric, LLC property. _ - The storm drains in the paved areas collect sheet flow precipitation and drain either to a pond adjacent to the Mt. Airy-Surry County Airport on the southwestern portion of the site or to a riser -type sediment basin on the southern portion of the site Stormwater that does not enter the storm drain system flows through curb drains to a swale east of the property that eventually enters Stoney Creek, or an intermittent stream on the southwestern edge of the property, around the pond adjacent to the airport and eventually joins Stoney Creek. The Altec facility receives stormwater runoff from -the, dja ent northeastern Pike Electric, LLC property. Paved driveways and parking areas surround'ng th6Altec"buildings have a storm drain system that drains to the south and southwestern portions ofit;h`e site. There.,are three terraced gravel staging areas on the southern portion of the site 1<\N The storm drains in the paved areas -collect sHeetflow runoff and drain either to a pond adjacent to the nt Mt. Figurey6-1 protides a site layout map showing the direction of stormwater flow on the property Potential Polidt'on Sources Raw, intermediate, final products are covered or inside buildings when feasible. The majority of chemicals are kept under cover at the facility. The diesel fuel station, the bulk liquid propane tank, and areas storing oxygen, nitrogen, carbon dioxide, and propane cylinders are located outdoors Aerial lift trucks are parked outdoors. Painted steel parts may be stored outdoors while raw steel is covered or indoors There are rock chutes and concrete flumes in the drainage areas with a high potential for soil erosion. Potential stormwater pollution sources may arise from sediment breakdown of gravel covered areas (total suspended solids); vehicle leaks (oil and grease); fueling operations (oil and grease, hydrocarbons) outside storage, and loading/unloading operations Records of materials used in these areas that represent potential contaminants are maintained as part of the annual air emissions reporting Altec Industries, Inc — Mt Airy, North Carolina Revision Date 08 August 2017 Pagel 6 - 2 Supersedes None Release Prevention Control and Countermeasures Plan requirement. In order to decrease or eliminate potential pollution, best management -practices have been implemented. Best Management Practices (BMPs) Best management practices (BMPs) have been implemented to prevent pollutants from entering stormwater discharges. BMPs are selected, installed, operated, and maintained in accordance with the requirement of the general permit The Chair of the Stormwater Team is responsible for ensuring necessary BMPs are implemented in a timely manner. Many baseline BMPs implemented at the facility, including visual inspections, spill prevention, and response, are discussed throughout this Plan General BMPs implemented at the Altec Mt. The inspections are meant to uncover conditions such as cracks or slow leaks which could cause breakdown or failures that result in discharges of chemicals to stormwater. The program is designed to prevent breakdowns and failures by adjustment, repair, or replacement of equipment. Equipment in the preventive maintenance inspection and testing program includes machining equipment, pipes; pumps; storage tanks; material handling equipment, and stormwater management devices (i.e., oil/water separators and storm drains). A record system for scheduling tests and documenting inspections in the preventive maintenance program has been implemented. Documented test results and corrective action are on file. The Building Maintenance Department maintains the preventive maintenance records. Visual Inspections Visual inspections are used to evaluate existing BMPs and to identify condition nth potential to cause contamination of-stormwater discharge Visual inspections schedules are listt6d, ;Section 7, Table 7-1. Spill Prevention and Response Developing spill prevention and response procedures is a criticaleementt of an effective SWPPP Section 3 identifies areas where spills may occur, material.li-andlmg p\rocedures, and spill control equipment. In addition, spill response procedures are discusssed4.Ihr ghouAlhe RPCC Plan Employee Training``jj A The purpose of SWPPP training is to teach personne <a all levels, -,of -responsibility the components and goals of the SWPPP Refer to Section 9 for Altec assacia trainnD Recordkeeping and Reporting , Incidents such as spills or other discharged, along ith,,Wer information describing the quality and quantity of stormwater discharges will be<tecAedit of the facility inspection and recordkeeping program are included in Section 7 E. STORMWATER MONITORI G PROEM Monthly Stormwater Inspektions \ a v i y In accordance withl th6e genera\stormwater permit, monthly site inspections are conducted by a stormwater team me— ,ner T!e inspections include observations and evaluations of BMP effectiveness, deficiencies, andcorectiveomeasures that will be taken Deficiencies are corrected within seven (7) days and must be dgcumentedL m -4h inspection report along with the dates and descriptions of any corrective actions or improvXementVttta were implemented. A copy of the monthly stormwater inspection form is included in Appendiia)V In addition, a stormwater team member, or his designee, shall conduct daily and/or weekly walkthrough inspections of areas to ensure that BMPs are continually implemented and effective. Structures that function to prevent pollution of stormwater or to remove pollutants from stormwater will also be specifically inspected Analytical Monitoring The general stormwater permit requires analytical (laboratory) monitoring of Outfalls 001 and 002. Altec Mt. Airy is required to conduct quarterly sampling of stormwater discharges. Altec Industries, Inc — Mt Airy, North Carolina Revision Date 08 August 2017 Page 16 - 4 Supersedes None Release Prevention Control and Countermeasures Plan The following parameters are followed for analytical monitoring: • Samples will be collected from a precipitation event exceeding 0.1 inches during a 24-hour period Precipitation events include rainfall and run-off from the melting of frozen precipitation. • Grab samples will be collected during the first 60 minutes of discharge. • Samples will be collected prior to discharge leaving or at the property boundary, or before discharge enters waters of the state on the property. • Examinations will include observations of color, odor, clarity, floating solids, settled solids, suspended solids, foam, oil sheen, or other obvious indicators of stormwater pollution. 4&1) Documentation will be maintained with the SWPPP if the facility is unable =to ycollect grab samples. Records will indicate if no rain event, or no "measurable" storm event occurreduring a monitoring period (including dates/times the outfalls were viewed and/or sampling- Wasp attempted). Acceptable documentation may include weather station data, local weather stVq AAdata, facility rainfall logs, and other appropriate supporting data If adverse weather conditions prevent the collection of samples, a substitute sample will be taken during a qualifying storm event ifi..t epY'ext-fib,itonng period. Adverse weather conditions are those that are dangerous or create inaoces ibilityfaa pe`isonnel, and may include such things as local flooding, high winds, electrical storms, o`\jluation1 h t otherwise make sampling impracticable, such as drought or extended frozen conditions A' Collecting Grab Samples for Analysis Sample Documentation ," Correct sample labeling and a complete ctiainvof cust d are critical to ensure proper handling and analysis by the laboratory Laboratories can, provide waterproof labels for each sample and a chain of custody form. The following mformatiop, hou'ld beecluded on the label and/or chain of custody. • Unique Sample or Log -Ni - AI,mples should be assigned a unique identification number Sample nu b re s may,'ON yy,combination of the date, material sampled and location where the sampleZ&en. Preservation Used - Any preservatives (and the amount) added to the sample should be recorded. The method of preservation should be indicated, including ice for temperature. • Analysis Required - All parameters for which the sample must be analyzed at the laboratory should be specified. • Comments - Any relevant information pertaining to the sample or the sampling site that may impact sample results should be recorded. This may include unusual weather conditions Altec Industries, Inc — Mt Airy, North Carolina Revision Date OS August 2017 Pagel 6 - 5 Supersedes None Release Prevention Control and Countermeasures Plan Observations regarding the quality of the sample (Le , excessive particulate, presence of oil films, color, etc.) should be noted in the sampling documentation and it should be determined if corrective action is required. Sample Packaging and Shipping If the samples are not hand -delivered to the laboratory, they should be shipped in a cooler with the chain of custody Caution should be taken in packing to ensure that glass containers do not break during shipping. Ice or a synthetic ice should be placed in the container so that samples maintain a temperature of 39.2°F (4°C) throughout shipping. Seal the container to prevent tampering. Most samples will not require any special transportation precautions except careful packaging to prevent breakage and/or spillage. If shipped by common carrier or sent through the U S mail, samples must comply with Department of Transportation Hazardous Materials Regulations. StormWater samples are not generally considered hazardous materials However, carriers should be con[. e8 to determine if transportation regulations apply Figure 6-2 provides a visual diagram of sam Ile packaging Sample Preservation and Holding Times N Preservation techniques ensure that the sample remains representati e oaf+he stormwater discharge at the time of collection. Since pollutants in the collected samples rn vNbe unstable,- samples should be analyzed immediately, or preserved to minimize changes between'the time of collection and analysis. Sample preservation techniques consist of lowering thete rature,pH> adjustment, and chemical fixation. Refrigeration is the most widely used techniquue,16\ ause i Zhao no detrimental effect on the sample composition, and it does not interfere with mdst analytic • methods. Refrigeration requires the sample to be quickly chilled to a temperature of 39 2 F 4 C). This e°bhnique is used at the beginning of collection in the field (placing samples in a cooler i ), dud g -shipment, and at the laboratory In addition to preservation techniques, maximum amount of time that sample Samples exceeding these holding timet repeated.
