Slnce the tract is mainly the existing NCDOT WW, most of the vegetation consists of roadside grasses on shoulders and
slopes. Within the R/W in one quadrant there are several species of hardwood trees and pines. No aquatic vegetation was
noted at the site.

Onslow Bridge #54 which carries SR 1231 (Banks Bridge Rd) over the New River.

The land uses adjacent to the bridge are residential, forested, and agricultural.

In addition to this completed application form, (MP-7) the following items below, if applicable, must be submitted in order for the application
package to be complete. Items (a) —(� are ahvays applicable to any major development application. Please consult the application
instruction booklet on how to properly prepare the required items below.

b. An accurate, dated work plat (including plan view and cross-sectional drawings) drawn to scale. Piease give the present status of the
proposed project. Is any portion already complete? If previously authorized work, clearly indicate on maps, plats, drawings to distinguish

c. A site or location map that is sufficiently detailed to guide agency personnel unfamiliar with the area to the site.

5.c. Describe the proposed construction methodology, types of construction
equipment to be used during construction, the number of each type of equipment
and where it is to be stored.
The existing eroded area on the left downstream bank will be isolated wifih an approved dewatering
measure, and then dewatered. The measures to dewater will be placed within the area of temporary
surface water impacts shown on the plan view drawing. All measures will be landward of interior Bent
2. Also, a turbidity curtain will be available and used as needed. After dewatering, the area around the
exposed water line will be stabilized, and then a mini-excavator with rubber tracks will access the site
from the upstream, western bank. Class II granite riprap will be placed along the bank per the plan
sheets. The riprap will be pushed into the sediment so that the rock is "keyed-in" along the proposed
new water's edge. After this line is established, sand fill material will be used to re-create the bank.
Class II granite riprap will be placed over the sand fill. The operation will be assisted by a long arm
excavator, operated from the roadbed or bridge deck. Also, several dump trucks will be utilized to haul
the riprap and sand. Some equipment may be stored on-site where there is sufficient shoulder width to
leave the equipment on-site. However, some equipment will likely be stored at NCDOT's maintenance
yard in Jacksonville, NC or Kenansville, NC.

As required by the NC Division of Coastal Management Regulations, you are being notified as an
adjacent riparian property owner that the North Carolina Department of Transportation (NCDOT)
plans to stabilize the northern slope of Onslow County Bridge #54 to address recent erosion. This
bridge carries SR 1231 (Banks Bridge Road) over the New River. The attached plan sheets and
application depict the proposed construction. Please indicate your position or viewpoint on the
proposed project and return the completed form and any comments to: Mr. Stephen Lane, Division
of Coastal Management, 400 Commerce Ave., Morehead City, NC 28557 or call (252) 808-
2808.
Should you have objections, your comments must be received within 10 days of receipt of this notice.
No response within 10 days will be considered the same as no objection if you have been notified by
certified mail.
